St.LucieUnit2DocketNo.50-389ReplytoNoticeofViolationInsectionReozt95-21RESPONSEA:1.REASONFORVIOLATIONFPLconcurswiththeviolation.TherootcauseofthisviolationwascognitivepersonnelerroronthepartofautilitylicensedoperatorwhofailedtorequirethatanIndependentVerification(IV)beperformed,inaccordancewithprocedure,forthepositioningofthepowersupplybreakertotheAtmosphericDumpIsolationValve.2.CORRECTIVESTEPSTAKENANDTHERESULTSACHIEVEDA.ClearanceOrder2-95-11-128wasremovedonDecember4,1995.ThepowersupplyswitchforMV-08-17,Breaker60957,wassubsequentlyrestoredtoitsnormalposition(On)underEquipmentClearanceOrder2-95-09-261onDecember6,1995.AnIndependentVerificationoftheswitchrestorationwasperformedatthattime.3~CORRECTIVESTEPTOAVOIDFURTHERVIOLATIONSA.OperationsDepartmentSeniorReactorOperators(SRO)werecounseledinwritingoftheneedformeticulousattentiontodetailwhengeneratingEquipmentClearanceOrders.B.Acrossfunctionalteamwillbeassembledtoreviewtheequipmentclearanceprocessandproviderecommendationsforimprovement'.ThisactionwillbecompletebyJune1,1996.C.Thiseventwillbeincorporatedintolicensedoperatorr~qualificationtraining.ThisactionwillbecompletebyJune30,1996'.FullcompliancewasachievedonDecember6,1995withthecompletionofitem2A,abov St.LucieUnit2DocketNo.50-389ReplytoNoticeofViolationInsectionReort95-213.CORRECTIVESTEPSTOAVOIDFURTHERVIOLATIONSA.Theutilitynon-licensedoperatorwhoperformedtheindependentverificationonvalveV09120wasdisciplinedinaccordancewithplantpolicy.B.AProceduralchangewillbemadetoAdministrativeProcedureAP-0010123,"AdministrativecontrolofValves,LocksandSwitches",tostrengthentheguidanceassociatedwithverifyingthelockedpositionofvalves.ThisactionwillbecompletebyJanuary31,1996.C.Thiseventwillbeincludedintooperatorrequalificationtraining.ThisactionwillbecompletebyJune30,19964.FullcompliancewasachievedonDecember11,1995,withthecompletionofitem2A,above.VIOLATIONC:TechnicalSpecification3.1.2.9requiredthatboronconcentrationbeverifiedconsistentwithshutdownmargininMode5withreactorcoolantsystemlevelabovethehotlegcenterlinebyuseoftheboronometerorbysamplingthereactorcoolantsystematafrequencydeterminedbythenumberofoperablechargingpumpsasshownonTable3.1-1.Contrarytotheabove,Unit2enteredMode5at3:10amonNovember27,1995,whichrequiredthatthesamplingperiodicitybeincreasedtoeveryeighthours.Sincethepastprevioussamplewasat1:00amonOctober27,thenextsamplewasdueat9:00amandeveryeighthoursthereafter.At11:45pm,operatorsdiscoveredthatthesamplinghadnotbeenaccomplishedattherequiredtimesonthedayandpeakshift.Chemistrywasnotifiedandthesamplewastakenandanalyzedat12:00pmonNovember2 St.LucieUnit2DocketNo.50-389ReplytoNoticeofViolationInsectionReort95-21RESPONSEC:REASONFORVIOLATIONFPLconcurswiththeviolation.TherootcauseofthisviolationwascognitivepersonnelerrorbyutilitylicensedoperatorswhodidnotstrictlyadheretotheAdministrativeProcedure(AP2-0010125)checksheetswhichdelineatetheboronsamplingsurveillancerequirementstobeperformed'eachshift.Duringthetimethattheboronsamplingrequirementswerenotmet,noChargingPumpswereoperableoravailablefordilution.ThisledtheoperatorstobelievethatsamplingrequirementshadnotchangedwhentheUnitenteredMode5.2.CORRECTIVESTEPSTAKENANDTHERESULTSACHIEVEDA.AReactorCoolantSystem(RCS)samplewastakenimmediatelyupondiscoveryofthiseventonNovember27,1995.Boronconcentrationwasfoundtobeconsistentwithshutdownmarginrequirements,andnoRCSdilutionhadoccurred.B.ADataSheet30,"UnscheduledSurveillanceTrackingSheet"wasinitiatedtoensurethatthecorrectsamplingfrequencywasbeingfollowed.3.CORRECTIVESTEPSTOAVOXDFURTHERVIOLATIONSA.Theutilitylicensedoperatorsinvolvedwiththiseventweredisciplinedinaccordancewithplantpolicy.B.St.LucieActionReport(STAR)951546wasinitiatedtoevaluatethecurrentTechnicalSpecificationrequirementsforRCSboronsamplingfrequencies.Changeswillbesubmittedasappropriatebaseduponthisevaluation.ThisactionwillbecompletedbyJune1,1996.C.Thiseventwillbeincludedintolicensedoperatorrequalificationtraining.ThisactionwillbecompletebyJune30,1996.4.FullcompliancewasachievedonNovember27,1995withthecompletionofitem2A,abov
