St.Lucie.Unit2DocketNos.50-389~~ReplytoNoticeofViolationInsectionReort95-05VIOLATION:10CFR50,AppendixB,CriterionIII,asimplementedbyFloridaPowerandLight(FP&L)TopicalQualityAssuranceReport(FPLTQAR1-76A),Revision38,requires,inpart,designcontrolmeasureswhichassurethesuitabilityofapplicationofmaterialsandpartsthatareessentialtothesafety-relatedfunctionsofthestructures,systems,andcomponents.Plantchangeandmodification(PC/M)package039-294,requiredthatoverloadheaterswiththemodelnumber123F114Cbeinstalledinmotorcontrolcenter(MCC)2A5,CompartmentNo.DR3,forBatteryCharger2AA.Contrarytotheabove,asofApril26,1995,designcontrolmeasuresdidnotassurethat'therequiredoverloadheaterswereinstalledinthat,overloadheaterswiththemodelnumber123F104CwereinstalledinMCC2A5,CompartmentNo.DR3forBatteryCharger2AAinsteadoftherequiredmodelnumber123F114C.TheinstalledoverloadheaterswerelessconservativethanthemodelnumberrequiredbythePC/M.RESPONSE:REASONFORVIOLATIONTherootcauseofthiseventwasthatprocesscontrolswhichwereinplacegoverningtheissuanceandinstallationofsafety-relatedcomponentsfailedtoensurethatdesignrequirementswerecorrectlyimplementedasspecifiedbythePC/M.AProcurementEngineeringgenericevaluationperformedin1991containedanuncleardispositionofmaterialinstock,resultingintwodifferentoverloadheatermodelsbeingstoredunderthesameMaterialandStock(MSS)identificationnumber.Genericevaluationsofthistypewereperformedonlyduring1991and1992.Electrical.MaintenancepersonnelfailedtoadequatelyresolveadiscrepancybetweenthepartsissuedfromstockandthosespecifiedbytheworkorderandPC/ St.Lucie.Unit2~~~DocketNos.50-389'ReplytoNoticeofViolationInsectionReort95-052~CORRECTIVESTEPSTAKENANDTHERESULTSACHIEVEDA.Operationsdeclaredthe2AABatteryChargeroutofserviceonApril27,1995.B.ElectricalMaintenancereplacedthethermaloverloadheatersinthe2A5MCCwithmodelnumber123F114C,andthe2AABatteryChargerwasreturnedtoserviceonApril28,1995.C.ASt.LucieActionReport(STAR)wasissuedwhichevaluatedthepastoperabilityofthe2AABatteryCharger.Thisevaluationconcludedthatthe2AABatteryChargerwascapableofperformingitsintendedfunctionduringthetimethethermaloverloadheaters(model123F104C)wereinstalled.D.TheEngineeringDepartmentreviewedallotherprocurementgenericevaluationsperformedduring1991and1992toverifytheaccuracyofmaterialdescriptions.Noadditionaldiscrepancieswerefoundtoexistbetweenmaterialinstockandtheitemdescriptionsaddressedbythosegenericevaluations.3.CORRECTIVESTEPSTOAVOIDFURTHERVIOLATIONSTheMaintenanceDepartmentwillreviseAP-0010432,"NuclearPlantWorkOrdersAdministrativeProcedure"toincludeadditionalverificationthatcomponentsinstalledintheperformanceofaPC/Mmatchthedesignrequirementasspecified.ThisactionwillbecompletedbyJuly31,1995.B.C.TheProcurementEngineeringgenericevaluationprocesshasbeenchangedsothatspecificMaterialandStock(M&S)identificationnumbersarenolongerreferencedingenericprocurementevaluations.EngineeringconductedtrainingforProcurementEngineeringpersonneltoemphasizetheimportanceofprovidingclearinstructionsinevaluationsregardingthedispositionofinstockmateria St.Lucie.Unit2DocketNos.50-389ReplytoNoticeofViolationInsectionReort95-05D.NuclearMaterialsManagementwilldevelopinstructionsforperformingpartnumberverificationsonfutureinstockdispositionsresultingfromEngineeringevaluations.ThisactionwillbecompletedbyOctober1,1995.E.Thiseventwillbeincludedintothequarterlyoperatingeventstrainingprogramformaintenancedisciplines.ThisactionwillbecompletedbyDecember31,1995.F.Atechnicalreviewteamwasformedtofurtherexaminethematerialcontrolprocessandproviderecommendationsforimprovement.TeamrecommendationstomanagementwillbeprovidedbyAugust31,1995.FullcompliancewasachievedonApril28,1995withthecompletionofitem2Babov C~
