.Re.St.LucieUni:t2DocketNos.50-389ReplytoNoticeofViolationInsectionReort93-05VIOLATION:DuringanNRCinspectionconductedonFebruary2-March5,1993,aviolationofNRCrequirementswasidentified.Inaccordancewiththe"GeneralStatementofPolicyandProceduresforNRCEnforcementActions,"10CFRPart2,AppendixC,theviolationislistedbelow:Unit2TechnicalSpecification(TS)6.8.1.arequiresthatwrittenproceduresshallbeestablished,implemented,andmaintainedcoveringtheactivitiesrecommendedinAppendixA,ofRegulatoryGuide1.33,Revision2,February1978.AppendixA,paragraph3.dincludesproceduresforEmergencyCoreCoolingSystem(ECCS)operationandshutdown.ProcedureOP2-0030127,Revision46,PlantCooldown-HotStandbytoColdShutdown,Section8.31,requiredtheSafetyInjectionTank(SIT)ECCSisolationvalvestobeclosedduringtheUnit2shutdown.Contrarytotheabove,onFebruary1,1993,whileUnit2wascoolingdowntoperformreactorcoolantpumprepairsinMode5,operationspersonnelleftthepreviously-closedSITisolationvalvesopenfollowingasurveillancetest.Asaresult,laterinthecooldown,theSITspassivelyinjectedapproximatelyfourpercentoftheirvolumeintothereactorcoolantsystem.Operatorsnotedunusualplantbehavior,but,didnotdetectthatthisinjectionhadoccurredforapproximatelytwoshifts.RESPONSEThereasonforthisviolationwaspersonnelerrorbyalicensedoperator.AcontributingfactorwasthereactorcooldownproceduredidnotclearlyspecifyaposttestvalvepositionfortheSITdischargevalves.2.CORRECTIVESTEPSTAKENANDTHERESULTSACHIEVEDa.OperationsclosedtheSITdischargevalvesanddeenergizedtheirmotoroperators.b.ThepersonnelinvolvedinthiseventhavebeencounselledbytheOperationsSupervisoronthe.importanceofconfigurationcontro St.LucieUnit2DocketNo.50-389ReplytoNoticeofViolationInsectionReort93-053.CORRECTIVESTEPSTHAT'WERETAKENTOAVOIDFURTHERVIOLATIONS4.a."ReactorPlantCooldown-HotStandbytoColdShutdown"procedures(1/2-0030127)havebeenmodifiedtoclearlydefinetherequiredpost-testpositionandrequireasign-offforeachstepofthepost-testsequence.b."SurveillanceDataSheets"(1/2-0010125ADatasheet10)procedureshavebeenmodifiedtoreferencethe"ReactorPlantCooldown-HotStandbytoColdShutdown"procedures(1/2-0030127)forappropriatepost-testvalveposition.FullcompliancewasachievedonFebruary2,1993withthecompletionofcorrectiveaction2.aabove.
