St.LucieUnitNo.1DocketNo.50-335ReltoNoticeofViolationInsectionReort93-24Violation:Unit1TS6.8.1.arequiredthatwrittenproceduresshallbeestablished,implementedandmaintainedcoveringtheactivitiesrecommendedinAppendixAofRegulatoryGuide1.33,Revision2,February1978.AppendixA,paragraph7.b,SolidWasteSystem,included"spentresinsandfiltersludgehandling."ThiswasfurtherimplementedonsitebySt.LucieprocedureOP1-0520020,Revision30,RadioactiveResinReplacement.Pertinentattributesfollow:OP1-0520020,Section4.0,LimitsandPrecautions,statedthataNuclearWatchEngineer[alicensedSRO]shouldcoordinateandsuperviseeachresindischargeoperation.OP1-0520020,AppendixL,ResinTransferForSpentResinTank,hadanoteto"ensurepositivecommunicationexists...~~OP1-0520020,Section4.0,required"Donotexceed....SRT[spentresintank)pressureof25psigorshippingcontainerpressureof10psigduringsluicingoperations.OP1-0520020didnotdiscussvalvemanipulationstoaddwatertotheresindischargepipeviaabypasslineiftheresinenteringtheshippingcaskwerejudgedtobetoodry.Contrarytotheabove,onDecember2,1993,thelicenseefailedtoadequately,.implement(follow)andmaintainOP1-0520020,Rev30,inthat:TheNuclearWatchEngineerwasnotpresentatthesceneduringthetransfer,norwasanotherpersonatthescenedesignated"incharge".2.Thecommunicationsarrangementwasnot"positive"inthat,awallphonewasusedwhichpreventedviewingofgagePI6644,spentresintankpressure,whiletheoperatorwasindirectcommunication.3.OperatorsdidnotmonitorGagePI06-44,whichdisplayedthe"[10psiglimit]shippingcontainerorcaskinletpressure."Consequently,thespentresintankproceduralpressurelimitof25psigandthePI06-44proceduralpressurelimitof10psigwereexceededduringsluicingoperations.4.ProcedureOP1-0520020hadnotbeenupdatedtocorrectpreviouslyidentifiedinadequaciesregardingaddingbypasswatertocontrolthedrynessoftheresinbeadslurry.Asaresult,operatorsmanipulatedvalvesoutsideoftheproceduretoprovideabypassflowpathfortheadditionalwaterdesire 'I!C ISt.LucieUnitNo.1DocketNo.50-335ReltoNoticeofViolationInsectionReort93-24Response:1.ThereasonfortheviolationwasthatutilityoperatorsfailedtoproperlyimplementandmaintainOperatingProcedures(OP)1&2-0520020"RadioactiveResinReplacement".2.Acompletedebriefwasperformedbythecrewconductingtheresintransferevolutionstoidentifyallproblemareasand'roposedcorrectiveactions.OP162-0520020havebeenrevisedtoaddresstheidentifieddeficiencies.A.TheNuclearWatchEngineerorOperationsSystemsSpecialistisrequiredtobepresentinthefieldwhentheactualresintransfertakesplace.B.SpecificprecautionshavebeenaddedtomaintainradiocontactbetweentheOperationsdepartmentandHealthPhysicsstaffduringtheresintransfer.C.Thespentresintankpressurelimitationwasincreasedtoagreewithactualfieldconditions.Thisincreaseinpressureiswithinthedesignofthesystem.Thepressurelimitationonthedisposalcontainerwasdeleted.Thislimitationisnolongerapplicabletotheresinfillheaddesign.D.ValvesrequiredtobemanipulatedforcontrolofslurryconsistencyhavebeenspecifiedinOP1&2-0520020.OP2-0520020wassuccessfullyimplementedwithallrevisedrequirementsinplaceonFebruary4,1994.3.Nofurthercorrectiveactionsaredeemednecessary..4.FullcompliancewasachievedonFebruary25,1994.
