Violation (388/83-19-03):
Weld inadequately prepared for ISI.
Complete PSI volumetric examination requirements were not met with regard to weld DCA-210-2-FW2 due to a need for additional surface preparation.
The licensee determined that further surface preparation would jeopardize minimum wall requirements.
The licensee requested relief from perform-ing 100% of the code required PSI examination for a number of welds, including weld DCA-210-2-FE2, and the NRC granted the relief via Supple-ment 6 to the SER.
Based on the NRC issuance of SER Supplement 6 and the relief which was granted, this item is considered closed.

Unresolved Item (388/83-19-07):
UT indications not adequately dispositioned regarding weld DCA-210-2-FW3.
The licensee Level III per-formed a supplementary ultrasonic examination of the questioned weld to determine the accuracy of the original PSI data.
The examination results indicated that the condition was geometric in nature, was reproduced at various locations around the pipe, and agreed with original data.
The licensee's Level III stated that the ultrasonic signals could not be finger dampened at the O.D. surface, but that the sound was detected at the O.D. using a pitch/catch technique.
Based on the inspector's review of the licensee's examination results this item is considered close (Closed)
Unresolved Item (388/83-19-10):
Radiographic indication not dispositioned.
The radiograph of weld DCA-210-2-FW2 showed an indication several inches long at radiographic film area 12 through 18.
The indica-tion was not identified nor dispositioned by the licensee.
The inspector was informed that the ID surface was visually examined and that the sur-face was polished to remove a sharp corner on the counterbore.
The weld was then re-radiographed.
The inspector reviewed the original films and the films made subsequent to surface improvement, and found a linear indication on both sets of films at the same weld location.
The reader-sheet representing the latest films identified the in'dication and attri-buted the cause to counterbore.
Based on the inspector's review, and his discussions with the licensee's Level III, this item is closed.
Lon itudinal Seam Welds NRC Inspection Report 50-387/86-05 described problems with the extent of PSI examinations performed on longitudinal weld seams in Unit 1 piping.
As followup to those problems, the licensee reviewed the status of Unit 2 piping.
The licensee has discovered that 16 recirculation system class
welds were omitted from the Unit 2 PSI/ISI program.
The inspector reviewed licensee records and documentation to ascertain that the welds were volu-metrically examined during the current refueling outage in accordance with ASME Section XI requirements and are included in the Unit 2 ISI program for the 1st inspection interval.
Included in the inspector's review were ultrasonic examination data regarding the welds, an'd the appropriate portion of the ISI program plan.
The inspector found that all of the 16 welds were ultrasonically examined in accordance with the applicable code and regulatory requirements.
The extent of the examinations involved the first 12 inches of seam 'adjacent to the intersecting circumferential weld.
The examinations were done by IGSCC qualified personnel in accordance with a procedure which was quali-fied at the EPRI NDE Center for the detection of IGSCC.
No violations were identified.
Inservice Ins ection ISI Pro ram The inspector reviewed the facility ISI program to ascertain compliance with ASME Code and regulatory requirements.
The Unit 2 program was developed by the licensee and is included in the licensee's controlled document system.
The program is intended to meet the ASME Code Section XI 1980 Edition through Winter 1981 Addenda in accordance with the requirements of 10 CFR 50.55a(b).
Procedures are in place to assure that appropriate modifications and design changes are incorporated in the program.
Program status and examination tracking are accomplished with the use of a computer, which is updated on a daily basis
during the current outage.
The licensee's ISI vendor (SWRI) supplies qualified NDE personnel for the performance of the examinations and evaluation of the results.
Other aspects of the program, such as the selection of items for examination, maintaining on-site control of original data sheets, and the scheduling of examinations, are controlled by the licensee.
No violations were identified.

ualit Assurance A / ualit Control C
Involvement The inspector interviewed personnel to ascertain the degree of involvement of QA/QC with ISI activities.
The licensee intends to expand its QA/QC involvement in ISI activities, and has added a Level III individual to the QA/QC staff.
Qualified licensee ISI specialists presently perform surveillance of ISI vendor activities.
No violations were identified.
6.
Exit Interview The inspector met with licensee representatives denoted in paragraph 1 at the conclusion of the inspection on August 29, 1986.
The inspector sum-marized the purpose and the scope of the inspection and the findings.
At no time during this inspection was written material provided by the inspector to the licensee.
