Thank you for your letter dated February 5, 1979, informing us of your assessment of the noncompliance identified in our letter dated January 5, 1979.

With respect to your resy." e, our inspector reviewed a radiograph reader sheet listing incomplete penetration on a Class 1 weld as being acceptable with no supporting records to document how it was dispositioned.

Even though the final radiograph does not clearly show this incomplete penetration, one could conclude without other supporting data that the discontinuity still exists and it is being masked by the root geometry and/or added thickness of the weld.

ASME Section III, 1974 Edition, paragraph NB-5320, Radiographic Acceptance Standards, states in part that " Welds that are shown by radiography to have any of the following types of discor.tinuities are unacceptable:

(a) any type of crack or zone of incomplete fusion or penetration." This was the basis for our item of noncompliance.

ENCLOSURE

RESPONSE TO NOTICE OF VIOLATION The response to the item of noncompliance identified in Appendix A to the NRC letter dated January 5,1979 is provided in the following paragraphs.

Contrary to 10 CFR 50, Appendix B, Criterion X, a

radiographic report on the root of the azimuth 150 feedwater zafe-end weld showed lack of fusion at the 6"-- 6 3/4" area and was marked " acceptable."

The actual root radiograph was not available to check the defect.

There was no documentation of the grind-out or weld repair.

The radiograph of the completed weld was available and judged to be acceptable by two Level 2 inspectors and one Level 3 inspector.

CORRECTIVE ACTION TAKEN AND RESULTS ACHIEVER The noncompliance involved the acceptance of a radiography report that identified a rejectable lack of fusion in the root passes of the weld in question.

This report was for "information only" and provided an in-process record for the root pass.

This record was not required either by the ASME Code or the design (G.E.)

installation procedure.

Because the integrity of the weld was verified based on the acceptance by three qualified RT inspectors of the radiographs for the completed weld, no further corrective action was necessary.

The discrepancy on the "information only" root pass report had no effect on the quality of the work perforned or the acceptability of the subject weld.

CORRECTIVE ACTION TO AVOID FURTHER NONCOMPLIANCE

Although a discrepancy was identified on one radiography report, the fact that the report in question was a non-essential document for inofmation only and that all " final" radiography reports were found to be acceptable lead to the conclusion that the noncompliance identified is an isolated occurrence that does not indicate a breakdown in the quality assurance program.
