Routine resident inspection of previously identified items,

tire prevention / protection, preoperational test program implementation controls, preoperational testing quality assurance, and plant tours. This inspection involved a total of 65 inspector-hours onsite by the NRC Resident Inspectors including 9 inspector-hours onsite during off-shifts

Results: Of the five areas inspected, four were found satisfactory, and one i
noncompliance was identified in the area of preoperational testing quality assurance. (Violation of Severity Level VI - Failure to obtain and verify corrective action, paragraph 5.b)

The inspector verified the control room, including cabinet interiors.
and the cable spreading room were void of excessive flammable material, had adequate temporary fire fighting equipment installed and the equip-ment had been frequently checked, as required.

Preoperational Test Program Implementation Controls i
j The inspector performed audits and detailed walkdowns of portions of the Reactor Building Ventilation (VR) and Standby Gas Treatment (VG) Systems

as followup on previously identified items of noncompliance. This audit
was performed on a sample basis using the most current revisions of documents available at the time of the audit.
These documents arc refer-enced in section 5.a of inspection report 50-358/80-24.

Findings The specific findings in this audit are too numerous to allow com-plete inclusion in this inspection report. A list of individual j
components of the two audited systems was supplied to licensee
management representing examples of items 1,2,C3 below. Two addi-i tional items, 4,C5 below, are included as specific examples which

- may or may not have generic significance.

(1) Several components listed in the Sargent and Lundy (SCL) Instru-ment Index were not located on the system control C Instru-mentation Detail (CCID) or Piping and Instrument Detail (PCID) drawings,

Several components shown on the PCID drawings do not have l
unique identifying system designations assigned, are not j
listed in the SCL Mechanical Department Valve List (MDVL)

and are not included in the CCCE System Index.

Ventilation. system damners and gravity shutters identified on the SCL PCID drawings were purchased and installed by the licensee's HVAC contractor. There was no SCL listing avail-able to confirm the "as installed" status of these components.

C&ID M-787-14 incorrectly identifies IFE-VG002A & VG002B as 1FE-VG045 & VG046. This discrepancy was identified to S&L as a code 2 punchlist deficiency on October 11, 1979 and was still outstanding in November, 1980.

C&ID M-787-15 incorrectly identifies the motor operator for dampers IVG01YA and IVG01YB as a single component - IFZ-VG031.
Actual installation has two motor operators for each damper (1FZ-VG031A, B and 1FZ-VG032A, B respectively).
These rela-tionships were correctly identified on C&ID M-787-6 and M-787-12, respectively.
These C&ID drawings (M-787-6,12 and 15 were all revised simultaneously (Revision A, dated March 17, 1978) by the same S&L personnel (Drawn, Checked, Approved and PE stamped).
These items are considered to be unresolved and will be followed up in future inspections (50-358/80-26-01)
No items of noncompliance or deviationn were noted.
5.
Preoperational Testing Quality Assurance
The inspector verified that the licensee program for QA/QC surveillance and audit of preoperational testing activities has continued to be im-plemented as described in previous inspection reports (50-358/80-14 and 50-358/80-21).
In particular, the inspector verified that revisions made to previously reviewed procedures conform to applicable standards and regulatory guidance; that audits und surveillance inspections per-formed since the last inspection of this area were performed in accordance with approved procedures as designated by surveillance schedules; that corrective action to resolve inspection and audit discrepancies has been adequate and timely; and that QA personnel added to the station staf f since the last inspection of this area meet minimum education, experi-ence and qualification requirements.

Findingg (1)
There were no resumes included in the training department records f or two QC technicians. This omission was corrected by the station training coordinator.
(2)
CG&E Field Audit Report #307 identified a deficiency (report item 2.c) in the implementation of SU.PRP.01, System Release and Turnover. This deficiency was assigned by the Quality Assurance and Standards Group (QA&S) to the Generation Construc-tion Department (GCD) for their resolution.
In their letter 80-151, dated August 7, 1980, CCD acknowledged the deficiency and

recommended a revision be made to EC. SAD.03 to resolve it.
(EC. SAD.03 is under the cognizance of the Electric Production
,

Department.) Subsequently, QA&S closed Field Audit Report
- 307 via surveillance Report #284 without any ferther action being taken to correct the identified deficiency. This QA

Program failure is an example of inadequate corrective action and is considered to be in noncompliance with 10 CFR 50, Appendix B, Criterion XVI and the CG6E Wm. H. Zimmer Quality Assurance l
l Manual, Chapter 16, Paragraph 16.4.2 which states in part that corrective action on construction site audite is obtained and verified.

6.
Plant Tours The inspector conducted frequent plant tours throughout this inspection period. The items listed below were identified by the inspector and the licensee is taking or has taken appropriate corrective action.
Excessive amounts of combustible materials were found in the Servf ce a.
Water Structure. Also, several gas cylinders were observed which were not properly supported.

Light bulbs were observed in cable tray 2114A.

lying on the floor in the reactor building.

A collection of beer cans and paper trash was observed in the Ril piping tunnel on the east wall, 570' elevation of the reactor building.

Unresolved 1+. ems Unresolved items are matters about which more information is required in order to ascertain whether they are acceptable items, items of noncompliance, or deviations. One unresolved item disclosed during this inspection is discussed in paragraph 4.a.

Management Interview The inspectors met with licensee representatives (denoted in paragraph 1)
at the conclusion of the inspection on December 31, 1980.
