Appendix A NOTICE OF VIOLATION Cincinnati Gas and Electric Company Docket No. 50-358 As a result of the inspection conducted on December 1-31, 1980, and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7, 1980), the following violation was identified:
10 CFR 50, Appendix B, Criterion XVI requires in part:
" Measures shall be established to assure tha' conditions adverse to quality such as failures, malfunctions, deficiencies, deviations, defective material and equipment, and nonconformances are promptly identified and corrected."
Wm. H. Zimmer NPS Q.ality Assurance Manual, Chapter 16, Paragraph 16.4.2 states in part:
"Eignificant conditions adverse to quality are reported to CG&E management and to appropriate levels of the contractor's management.
Corrective action is obtained and verified where required."
Contrary to the above, The Quality Assurance and Standards Group, CG&E, failed to obtain and verify corrective action taken on a procedure imple-mentation deficiency identified in CG&E Field Audit Report #307 prior to closeout of the Audit Report.
This is a Severity Level VI violation, (Supplement I).
Pursuant to the provisions of 10 CFR 2.201, you are required to submit to this office within twenty-five days of the date of this Notice a written statement or explanation in reply, including for each item of noncompliance:
(1) corrective action taken and the results achieved; (2) corrective action to be taken to avoid further noncompliance; and (3) the date when full com-pliance will be achieved. Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted under oath or affirmation.
