This refers to the routine inspection conducted by Mr. R. A. Paul of this office on November 13 - 16,199b, of activities at Davis Besse Nuclear Power Station authorized by NRC Operating License No. NPF-3 and to the discussion of our findings with Mr. J. Polyak and others of your staff at the conclusion of the inspection on November 16, 1990.
The enclosed copy of our inspection report identifies areas examined during the inspection.
Within these areas, the inspection consisted of a selective examination of procedures and representative records, observations, and interviews with personnel.
No violations of NRC requirements were identified during the course of this inspection.
However, we are concerned that events involving apparent weaknesses in prejob review, planning, and communications and problems associated with recognizing potentially significant radiological problems continue to appear in your Radiation Awareness and Potential Condition Adverse to Quality reports.
Their recurrence suggests the possibility of a more pervasive weakness such as technician inexperience that may not be effectively addressed by your corrective actions for the individual events.
We request that you inform us within 30 days of the actions that have been taken or plan to take regarding this matter. We will review this matter further in subsequent inspections.
