NRC Inspection of the Trojan Nuclear Plant-This refers to the inspection conducted by Mr. R. C. Barr and Mr. J. F. Melfi of this office from June 18, 1989 through July 29, 1989 of activities n
authorized by NRC License No. NPF-1, and to the discussions af our findings

held by Mr. Barr and Mr. Melfi with Mr. Yundt and other members of your staff
. periodically'during the inspection.

. Areas, examined during this inspection are described in the enclosed inspection
,
report. Within these areas, the inspection consisted of selective
,
!
examinations of procedures and representative records, interviews with
'
personnel and observations by the inspectors.
'
. Based on the results of thi.s inspection, it appears that certain of your.
-
activities were'not conducted in full compliance with NRC requirements.,
,
Specifically,:the' inspection identified that (1) Engineered Safety Feature components (CCW/RHR heat exchangers) had been operated outside normal design limits,'(2) Operations Management was aware that an off-normal event had

. occurred and failed to research the event adequately, and (3) procedures were not changed once the operators recognized the evolution could not be conducted as written.
This single event emphasizes the following previously identified deficiencies at your facility:
utility management does not aggressively pursue and correct problems, compliance with procedures is inconsistent, and the quality of safety evaluations is inconsistent.
Additionally, the enclosed notice includes violations from inspection report 89-05, which demonstrated similar work control problems in the maintenance area and has been previously discussed in a management meeting.
Your response to this Notice is to be submitted in accordance with the provisions of 10 CFR 2.201 as stated in Appendix A, Notice of Violation.
The responses directed by this letter are not subject to the clearance procedures of the Office of Management and Budget as required by the Paperwork Reduction Act'of 1980, PL 96-511.
In accordance with 10 CFR 2.790(a), a copy of this letter and the enclosure will be placed in the NRC Public Document Room.
