Insoection on flovember 6-8,1979 (Report flo. 50-344/79-22)

Areas Insoected: Routine, unannounced inspection of employee training, response to IE Bulletins and Circulars, and followup on previous inspection items; also included independent inspection effort.

The inspection involved 34 inspector hours onsite by two flRC inspectors.

Results: Of the two areas inspected, one noncompliance item was identified in one area (infraction - failure to take followup actions on unacceptable surveillance test results - Paragraph 4).

Followup on Previous Insoection Findings (78-12-02, closed) Reactor Ve:sel Level Control during maintenance activities: The inspector observed during a visit to the control room that the modified standpipe was in use, with a TV camera providing the control room operator a direct reading of reactor vessel water level.

Training The inspector reviewed the overall training activities for non-licensed employees and general training for all employees.

Included in this was a review of records for several permanent and temporary employees in the area of general employee training and a system checkout completion record for a non-licensed employee. The licensee's program met or exceeded the requirements and recommendations of ANSI N18.1-1971 and Appendix A of 10CFR 55 as required by Section 6 of the technical speci-fications.

No deviations. or items of noncompliance were identified.

IE Bulletin / Circular Followup The inspectors followed up on licensee actions related to recent IE Bulletins and Circulars.

The Bulletins and Circulars had been forwarded to and reviewed by cognizant memberc of the licensee's organization.

Inspection findings relating to the bulletins and circulars reviewed during the inspection are as follows:

IE Bulletin 79-13, Cracking in Feedwater System Piping (open):

Inspection Report No. 79-12 discussed inspector review of the non-destructive examinations performed by the licensee and stated that IEB 79-13 would remain open pending receipt and review cf the required written response. The licensee's response, dated August 939

-2-28, 1979, was reviewed in-office subsequent to the inspection. The response discussed one indication on weld FW145 which required ex-tensive evaluation and stated that a structural analysic of this weld would be performed. A licensee representative

... acted by phone stated that additional ultrasonic tests had confirmed an acceptable indication thickness of.08", and that the structural analysis was in progress.

This bulletin will remain open pending receipt and review of the structural analysis report.

IE Bulletin 79-15, Deep Draft Pump Ceficiencies (closed):

The licensee's response dated August 29, 1979, summarized operating and maintenance history of timilar pumps at Trojan; in particular, service water and fire protection pumps.

Pump surveillance records and performance trend data reviewed by the inspector confirmed the acceptable performance described in the licensee's letter.

During review of surveillance records for the

"C" Service Water Pump, the inspector noted that the pump discharge pressure (30 psi) recorded during the 9/30/79 test did not meet the acceptance criterion (30.4 psi) indicated on the data sheet, although the completed data sheet had been signed off by an operator, shift supervisor, and responsible members of the technical support staff. Consequently, the required actions for unacceptable test results were not taken as specified in Section II.B.4 of QAP-11-1. This was identified to the licensee as an item of noncompliance (79-22-01).

Retest of the pump following the inspection verified satisfactory pump performance.

IE Bulletin 79-17, Pipe Cracks in Stagnant Borated Water Systems (open): The licensee's 30-day response dated August 24, 1979, provided initial information required by the bulletin and stated that to date no stress corrosion cracking had been identified in safety-related stainless steel piping systems at Trojan. The reply also stated that an inspection program would be implemented as outlined in the IEB, with a report submitted to flRC upon comple-tion.

Licensee representatives stated that the inspection program had been completed with no problems identified, and that the report was being prepared. This Bulletin will remain open pending receipt and review of the licensee's response.

IE Bulletin 79-18, Audibility Problems Encountered on Evacuation of liigh-floise Areas (open): Actions were proceeding as outlined in the licensee's reply dated September 19, 1979.

This bulletin will remain open pending completion of the licensee's actions.

IE Bulletin 79-21, Temperature Effects on Level Measurements (closed):

The inspector reviewed the licensee's reply dated September 14, 1979, and verified through review of related calibration records that the steam generator low-low level trip points had been changed from 5 to 15 percent as described in the licensee's letter.

The inspector also observed that the identified correction curves had been placed in the Trojan Control Room Operating Curves and Tables Reference Manual, and that the appropriate control room indicators had been marked to caution the operators about possible inaccuracy under post-accident conditions.

i IE Bulletin 79-23, Potential Failure of Emergency Diesel oenerator Field Exciter Transformer (open): The licensee's reply dated October 11, 1979, stated that a detailed field inspection had verified that the potential for high circulating currents did not exist at Trojan. This Bulletin remains open pending the licensee's performance of a 24-hour load test, which is scheduled to be conducted during the next refueling outage.

IE Bulletin 79-24, Frozen Lines (open): The licensee's reply dated October 23, 1979, described heat tracing and other temporary modifi-cations to be installed by November 25, 1979.

Discussions with licensee representatives and observations during a plant tour showed work to be in progress. This bulletin will remain open pending completion of the licensee's identified actions.

IE Circular 79-04, Loose Locking Nut on Limitorque Valve Operators (closed):

Licensee representatives stated that all affected valve operators had been inspected for tightness and staking and that a revision had been made to maintenance procedure M-12-5 to include inspection of locking nut tightness in the regular 3-year preventive maintenance.

IE Circular 79-05, Moisture Leakage in Stranded Wire Conductors (open):

Licensee representatives stated that this Circular was still under review.

IE Circular 79-12, Potential Diesel Generator Turbocharger Problem (closed):

Review of licensee procedures showed that MP-12-7 and 01-5-1 had been revised to incorporate the actions recommended by the Circular.

IE Circular 79-17, Contact Problem in GE Metalclad Circuit Breakers (closed): The inspector examined a memorandum to the General Manager which stated that a survey had shown none of the SB-12 switches are installed in the Trojan facility.

IE Circular 79-18, Target Rock Safety / Relief Valves (closed):

No Target Rock Safety / Relief Valves are used at the Trojan facility.

-4-IE Circular 79-19, Loose Locking Devices on Ingersoll-Rand Punp Impellers (closed): The inspector noted that a Part 21 report submitted by Portland General Electric regarding containment spray pumps had partly prompted the issuance of this Circular.

Related modifications had previously been made to core spray and residual heat removal pumps.

The other experiences outlined in the Circular had been reviewed by the licensee. A survey conducted by the licensee concluded that of fourteen other types of safety-related pumps, two types should be modified to provide a positive lock on the impeller nut, these being the component cooling water ilakeup pumps and the service water booster pumps. A Request for Design Change (RDC-089) had been initiated to accomplish these modifica-tions.

IE Circular 79-20, Failure of GTE Sylvania 5U12 Relays (open): The licensee stated that no GTE Sylvania SU12 relays are installed in the Trojan plant; however, similar relays made by a GTE subsidiary are in use, and the licensee was obtaining information which would indicate whether these are subject to the same failure mechanism.

Exit Interview

The inspectors met with licensee representatives (denoted in Paragraph 1) at the conclusion of the inspection on flovember 8,1979. The scope and findings of the inspection were discussed, including one apparent item of noncompliance (Paragraph 4). This item was discussed further by phone with licensee management following the inspection.
