APPENDIX A Portland General Electric Company 121 S. W. Salmon Street Portland, Oregon 97204 Docket No. 50-344 fl0TICE OF VIOLATION Based on the results of an NRC inspection conducted on November 6-8,' 1979, i t appears that certain of your activities were not conducted in full compliance with NRC requirements as indicated below. This is an infraction.
Paragraph 2.C (7) of the Trojan facility license, ?!RC License No. NPF-1, requires activities to which a quality assurance progran is applicable (except for piping supports) to be conducted in accordance with the Quality Assurance Program for Operation.Section II.B.4 of Trojan Quality Assurance Procedure OAP-11-1 requires test results to be re-viewed and specifies actions to be taken when test results do not neet acceptance criteria.
Contrary to these requirements, the test results for a periodic operating test of the "C" service water pump performed on September 30,1979, were approved by responsible reviewers even though recorded pump discharge pressure was less than the specified minimum, and the actions specified for unacceptable test results were not taken.
