SUBJECT: WOTICE OF VIOLATION (NRC INSPECTION REPORT NOS. 50-008/88-16 AND 50-339/88-16)

This refers to the Nuclear Regulatory Commission (NRC) inspection conducted on May 14 - June 10, 1988, at the North Anna Power Station. The NRC letter dated June 30, 1988, transmitting the above raferenced inspection report indicated that a potential violation was being reviewed for possible escalated enforce-ment action (violation 339/88-16-02) involving operabihty of the emergen'c diesel generators. The violation with pertinent references and elements to be included in your response is described in the enclosed Notice of Violation (Notice).

In our review of this event, we considered information presented during the Enforcement Conference on July 6, 1988, and in Licensee Event Report (LER) 339/88-04. We also considered that two outside sources of AC power were in service, and one or both emergency diesel generators could have been made operable on relatively short notic Nevertheless, the inoperability of emergency power source 3 is of concern no matter how short the time period involved. Nonnally such a violation is categorized as Severity Level II Since the plant staff had taken some action based on NRC Information Notice 87-41 and INP0 SOER 14-87 to prevent the problem that caused one of the emergency diesel generator to be inoperable, this violation was categorized as a Severity Level I While the actions taken were appropriate; they were insufficient. We remain concerned about your program to use industry experience to develop plans and programs to preclude similar events at your facilities. Sufficient general information was available that could have enabled you to develop specific steps to prevent this violation based on the events described in NRC Information Notice 87-41 and INP0 SOER 14-87. Past experience indicates that additional steps are required by Virginia Electric and Power Company management to develop l and implement an improved operational feedback program. Should shortcomings in your operational feedback program result in future violations of significant regulatory concern, such violations will be seriously considered for escalated )
