1 : ..', ( Attcchm:nt to:- L OCAN029301 page 1 Description of' Corrective Measures and Schedule for Completion: The licensee's failure to demonstrate the capability to perform accurate dose assessments in a timely manner from the_ control room following and un-monitored radiological release was identified as a weakness (313/9227-01;368/9227-01).

Response the root cause for the lack of proficiency with the Radiological Dose Assessment Computer Systems (RDACS), which was observed during initial dose assessment in the control room, has been determined to be procedural instructions which are not appropriately human factore The dose assessment procedures are not currently designed to account for the irregular frequency of their use (in comparison to procedures used in routine day-to-day activities).

Additionally, our root cause analysis included a general evaluation of the format of other procedures (e.g. , Emergency Classification, Emergency Radiation Team, Emergency Medical Team) to determine if similar format weaknesses were present elsewhere in the progra It has been determined that problems of thir, nature are not prevalent in other procedure serie This conclusion is based primarily on a review of drill performance results in which those procedures were implemented and utilized by emergency responder Remedial training was administered on December 10, 1992, prior to the end of the inspection, to the three dose assessment personnel-involved in the evaluated simulator drill A supplement to the existing procedures has been developed and implemente This supplement provides a condensed set of guidelines to aid users in performing dose assessments- in accordance with the current procedure forma Emergency planning is currently administering remedial training to dose assessment personnel to improve their skills for projecting the effects of un-monitored releases in general, including contaminated steam release This remedial training consisted initially of a required reading assignment which has been issued to members of the Dose Assessment Tea This required . reading assignment is currently being followed-up by a series of practical factors sessions for the team members. These sessions require'a demonstration of dose assessment skills and will be completed by-March 31, 199 %

The dose assessment procedures will be reformatted. The format will be selected by a task force comprised of representatives from the Dose Assessment Team, Dose Assessment Supervisors, - and the Emergency Planning Department. _The goal of the task force will be to produce a set o' dose ~ assessment instructions and guidelines that incorporate effective human factor technique A prime consideration of the task-force will be that the dose assessment task is performed on an irregular basis.
