Inspection during the period of January 6-10, 1986 (Report No. 50-312/86-01)
Areas Inspected: This routine unannounced inspection consisted of a review of open items, and the program plan,' procedures, records pertaining to the Rancho Seco inservice testing program for pumps and valves. The inspection involved a total' of 44 hours5.092593e-4 days <br />0.0122 hours <br />7.275132e-5 weeks <br />1.6742e-5 months <br /> onsite by one NRC inspector. During this inspection, inspection procedures 92701.and 92702 were covered.
Results:
In the areas inspected, no violations of NRC requirements were identified.

- The subject instruments have been used to record official surveillance data [ bearing oil cooler discharge temperatures], in order to monitor and evaluate bearing cooler performance, without any check of calibration since 1971 [per calibration stickers]. While these temperatures are not being recorded for verification against pump or cooler acceptance criteria, it has been determined that the data recording is required in official surveillance procedures to aide in the detection of biofouling in.the
,
- applicable coolers.
The accuracy of the subject instruments should have been verified and documented upon initial use in surveillance procedures, at least more recent than 1971.
It was noted that there was no documented evidence that data takers questioned the accuracy of the subject instruments.
Based on the above findings, the licensee representatives stated a review of the calibration requirements for the subject added instruments would be performed, along with a look at finding a better method of notifying procedure writers of new licensee commitments.
This item will remain open until the licensee has performed the above identified actions.

(Open) Unresolved Item No. 50-312/85-23-03: Adequacy of IST Implementing Procedures The licensee response by letter of September 3, 1985, stated in
.
part, "All surveillance ~ procedures will be revised to require the documenting of calibration data for all instrumentation,.other than control room instrumentation.... prior-to January 1, 1987".
At the time of this inspection, the licensee was preparing a guide for procedure writers to ensure the issuing of adequate changes aud revisions to IST implementing procedures.
This guide would ensure that the latest calibration requirements, licensee's commitments and positions were incluced in curveillancetprocedures.
This item will remain open until the' licensee has. performed the above identified actions.

(Closed) Notice of Violation No. 50-312/85-23-04: Failure to Test Additional Valves After Failure of Valves and Failure to Retest Valves After Repair The licensee response by letter.of September 3, 1985, stated in part, "As a result of these findings, the pest. records for the valves listed above were reviewed.
It is apparent that a lack of proper review and corrective action was responsible for these and other violations. The Safety and Relief Valve Setpoint Verification

The inspector reviewed-the latest Safety and Relief. Valve Setpoint Verification Test Procedure " Maintenance Testing (MT) 006",
revision 2 issued on September 20, 1985, and found it appeared.to provide the additional instructions necessary to resolve the

concerns addressed in this notice of violation.

Rate Tests The licensee is tracking this item and has assigned its review to one of-their staff. The licensee representative has performed an initial review of the subject notice, and believe that changes to testing procedures will be required prior tc the next containment testing, which should be performed in early.198'
