Summary:

Inspection of March 12, 1982 (Roport No. 50-312/82-09)

This was a special safety inspection conducted at Burlington Northern Airfreight at 2465 Wyandotte; Mountain View, California to observe a shipment of radioactive material.

On the evening of March 11, 1982, a Burlington Northern Airfreight employee notified the NRC Region V office that a fifty-five gallon drum from the Rancho Seco Nuclear Generating Station was observed to have a perforation that was about the size of a nail hole.

The employee reported that there was no observable leakage from the container.

The March 12, 1982 safety inspection included radiation surveys and contamination wipes, examination of packaging conditions, package marking and labeling, a review of shipping records, and discussions with licensee and Burlington Northern Airfreight representatives.

Two inspector hours were expended g,

in this effort by one NRC inspector.

On the evening of March 11, 1982, NRC Region V was notified by Mr.

Bob Wilke, Night Operations Supervisor at Burlington Northern Airfreight,

that a fifty-five gallon drum from the Rancho Seco Nuclear Generating

i Station was observed to have a perforation that was about the size

of a nail hole.~ He also stated that there was no observable leakage from the container.

Inspection:

A special safety inspection was conducted on March 12, 1982 at Burlington Northern Airfreight; 2465 Wyandotte; Mountain View, California, to observe the punctured container. The inspection included radiation surveys and contamination wipes, examination of packaging conditions, package marking and labeling, a review of shipping records, and discussions with licensee and Burlington Northern Airfreight representatives.

(a) General Packaging Conditions The package was a fifty-five gallon drum, which appeared to be in very good condition with the exception of a small puncture located about 8 inches above the bottom surface of the drum.

The puncture was about the size of a nail hole, and the area was slightly indented.

See Appendix A, photos 1 and 2.

There was no observable leakage, and it appeared that the contents nearest the puncture was a solid material.

Burlington Northern Airfreight personnel placed tape over the hole when the package arrived. The inspector removed the tape during the inspection and replaced it at the completion of inspection activities. See Appendix A, photo 3.

The bolt on the sealing ring was equipped with a wire and lead security seal to assure that the drum was not opened during transport.

-2-(b) Radiation Surveys The maximum radiation reading at three feet from the surface of the container was 1.0 mr/hr. The maximum radiation reading with the instrument probe at the surface of the container was 170 mr/hr.

This maximum dose rate was found in a highly localized area. Radiation levels with the instrument probe at the surface of the container were generally between 30 to 80 mr/hr. These radiation measurements were made with a Region V Eberline E-520 instrument, NRC #007907, calibrated on March 9, 1982 and due for recalibration on June 9,1982.

An HP-270 G-M probe was used with the instrument for these measurements.

The radiation levels were within the D.0.T. limits of 200 mr/hr

at any point on the external surface of the package and 10 mr/hr

at three feet from the surface of the package.

(c) Contamination Surveys Four wipes of about 300 square centimeters each were taken to check the surface of the package for removable radioactive contamination.

One wipe was taken from the top of the drum. Another was taken around the punctured area while the tape was in place. A third wipe was taken around the punctured area with the tape removed.

A final wipe was taken on the opposite side of the drum.

The wipes were checked with the NRC #007907 instrument and an HP-260 pancake probe at the time of the inspection.

No removable radioactive contamination was detected.

(d) Package Marking and Labeling The drum was labeled with two completed D.0.T. Radioactive Yellow III labels. The contents were indicated to be 0.0071 curie of Co-58, Co-60, and Mn-54 with a transport index of 6.

The drum was also labeled with a " Danger - Peligro" label.

The words " Caution, Radioactive Material" and the radiation symbol i

were stencilled on the drum. The drum was also marked with the words, " USA D0T-7A, Type A" and "255 lbs."

(e) Shipping Records A copy of the shipper's certification is attached as Appendix B to this report.

-3-The Burlington Northern Airfreight Airbill, which was included with the paperwork for this shipment, was number SMF 145501355, dated March 11, 1982. The shipper was Rancho Seco Generating Station; 14440 Highway 104; Herald, California 95638. The consignee was Babcock and Wilcox; P. 0. Box 1260; Lynchburg, Virginia 24505. The shipment was received from W. A. Wilson at Pancho Seco on March 11, 1982 at 2:35 p.m. by driver Mark E. Call.

This was not an exclusive use shipment. Other.information given on the airbill was also indicated on the. shipper's certification.

Shipping records also included.a.SMUD " Radioactive Material Shipment Form" numbered 82-1, a SMUD survey sheet, and instructions to the carrier.

Mr. Christman of Burlington Nort.hern Airfreight stated that driver Mark E. Call works for Valley Parcel. Mr. Christman also stated that the package was taken from Rancho Seco to a depot in Sacramento where it was transferred to a vehicle bound for the bay area.

(f) Other Inspection Findings Mr. Christman stated that Valley Parcel does not normally nail boards together to make bracing structures for their shipments.

However, he remarked that he did not know whether this shipment was braced, since ha was not present when it arrived.

Licensee representatives arrived at the Burlington Northern Airfreight facility at noon on March 12, 1982. They placed the fifty-five gallon drum in a wooden overpack, and loaded the package onto the bed of a SMUD pickup truck.

The truck was placarded, and the wooden overpack was marked with D0T Yellow III labels, the weight of the package, and " Caution, Radioactive Material" signs.

The maximum radiation level in the cab of the vehicle was 0.7 mr/hr as measured with the NRC #007907 instrument with the HP-270 probe. The licensee representatives stated that they would return to the Rancho Seco Nuclear Generating Station and that the shipment would be repackaged.

4.

Conclusion Since the puncture in the fifty-five gallon drum could have occurred during transport, and since no leakage of radioactive materials resulted, there was no item of noncompliance with N.R.C. or D.O.T. regulations observed during this inspection.
