Klingler, Enforcement Specialist, I&E Discussion and Results NRC Region I management summarized recent activities which involved problems with control of station activities at Unit 2. These activities ae summarized in the attachments and are discussed in Combined Inspection Report 50-272/83-16 and 50-311/83-15, Inspection Report 50-311/83-18 and Combined Insp~ction Report 50-272/83-13 and 50-311/83-1 The most significant concern involved blocking of the RCS vent path. The licensee indicated that the main cause of this violation was that the maintenance supervisor did not follow the shift supervisor's instructions and the maintenance supervisor has been disciplined. The licensee also noted that their policy is now to avoid multi-step work orders (such as to fabricate and install equipment) to ensure plant conditions are satisfied when work orders are approved. Additionally, the licensee has counseled all maintenance supervisors of this problem and will provide written direction to the Maintenance Departmen The licensee attributed the problems with the other activities, in particular, the Pressurizer Overprotection Protection (POPs) system challenge and the containment airlock problem, to personnel and procedural error. The affected test procedure which led to the POPs challenge has been revised and a standing order was issued to ensure that containment airlocks are tagged out when declared inoperable. The licensee noted that during the POPs system challenge on June 17, 1983, the RCS was not solid as reported in LER 83-029. From a broader perspective, the licensee indicated that they are trying to get more operationally experienced people into the maintenance and technical departments. Additionally, they are working on outage management to improve control of activities.

Region I management acknowledged and commented on the licensee's description of the events and corrective actions. The licensee will provide written documentation of these corrective actions in response to the Notice of Violations.
