Subject: Response to Notice of Violation Gentleme In response to the Notice of Violation received in Reference (b), Maine i Yankee has taken the following actions to achieve compliance and to prevent l future occurrences. The Notice of Violation contained in Reference (b) is ,

area in which the intensity of radiation is at such levels that a major i portion of the body could receive in any one hour a dose in excess of 100 mrem shall be barricaded and conspicuously posted as a high radiation area, Contrary to the above, on April 7,1987, whole body dose rates in excess of 100 mR/hr were present on the -2 foot elevation, adjacent to the access area to loop #1 in the Containment Building, an6 were inadequately posted and barricade Contrary to the above, on April 8, 1987, between 1230 and 1410, access to Reactor Coolant Pump #2, an area with whole body radiation intensities in excess of 100 mR/hr, was not barricaded nor  !

I Contrary to the above, on April 10, 1987, at approximately 1005, access to the refueling cavity, an area with whole body radiation intensities in excess of 100 mR/hr, was not barricaded nor conspicuously poste The above multiple instances of failure to adequately control high  ;

radiation areas collectively constitute a Severity Level IV violation (Supplement IV).

Maine Yankee Resoonse Maine Yankee took the following immediate action regarding maintaining the proper posting and barricading of high radiation areas in Containment  !

identified ~1n a, b, and c abov The area underneath the hot spot which produced dose rates greater than 100 mR/hr at the -2 foot elevation adjacent to Loop #1 access in i containment was immediately barricaded and posted at a point where whole body dose rates were less than 100 mR/h Although the access to the #2 RCP had been properly posted and barricaded, the workers who were continuously moving in' and out of the area improperly took down the barricade to provide easy access and failed to replace it each time they exited the high radiation area. Upon identification of the barricade being down, the barricade and posting was immediately repositioned. The technicians covering the work area were instructed to maintain the barricade across the stairway. The workers and their foremen were also instructed about the need to keep radiological barricades in place at all time The access to the refueling cavity had also been properly posted and barricaded. When workers exited the refueling cavity they took down )

the rope barrier and failed to reposition i The workers, their i supervisors, and the radiological controls technicians covering the work were all instructed on the importance of keeping radiological barricades in place at all time Full compliance was achieved on April 10, 198 To prevent a recurrence, written job coverage instructions, which included I the requirement for surveillance and maintenance of postings and barricades, i were issued to all radiation control work stations. More frequent-supervisory inspections of containment work locations have been conducted, especially i the areas referenced above, to verify the location.and posting of barricades l for high radiation area I i
