Valve Wall Thickness Verification The valve wall thickness verification program report was presented to the inspector for audit. An audit of the documentation and pro-cedures revealed the following deficiencies:
The thickness of valve bonnets, cover plates, and nozzles a.
adjacent to the veldend preparations had not been verified.
b.
The engineering justification for the wall thickness of Crane Co.14 inch,1500 lb., check valves CFV-1, 2, and 3 did not meet the requirements of RO:II letters of June 30, 1972, and February 16, 1973.
c.
Ultrasonic thickness measurements could not be obtained at the welded connection between the lower base to upper flange of
the 2-1/2"-2500 lb., Dresser Industries, pilot operated relief valve.

In the management interview FPC stated that additional measurements would be made of the valve bonnets, cover plates, and nozzle adjacent to the weldend preparations.
JAJ presented their engineering justification for the Crane. Co., -14 inch,1500 lb., check valve

CFV-1, 2, and 3 and FPC stated that these valves would be repaired

by welding to meet the requirements of the purchase order.
It was stated in. the interview that B&W had informed FPC they were, awaiting
,
RO evaluation of an engineering justification on an identical 21/2
inch, 2500 lb., Dresser Industries, pilot operated relief valve
'
installed at another site.
.
Subsequent to the inspection FPC was informed by telephone that their failure to verify the thickness of valve bonnets, cover plates, and nozzles adjacent to the weldend preparations was a violation.
They were also informed that the weld repair of the 14 inch,1500 lb., Crane Co., check valves and the engineering justification of the 21/2 inch, 2500 lb., Dresser Industries, valve would be un-resolved items.
