Dear Mr. Olivier:

On April 20,1998, the NRC completed an inspection at the Pilgrim Nuclear Power Station in Plymouth, Massachusetts. The enclosed report presents the results of that inspection.

During the seven weeks covered by this inspection period, the conduct of activities at the 5 Pilgrim Facility was generally characterized by safe plant operations. Three violations of NRC requirements were identified that are cited in the enclosed Notice of Violation. One violation involved the failure of your staff to identify multiple degraded equipment problems for correction through your site quality assurance program. The other two violations involved safety related maintenance activities conducted during power operations that resulted in damaged safety related equipment. In one case, significant rework was required on a core spray pump motor involving additional radiation exposure and equipment outage time. In the second case, a salt service water system pump shaft failed in service after a repair was mde. These events, combined with the installation of a wrong relay due to a work control arror as documented in NRC Inspection Report No. 50-293/98-01, indicate problems with the implementation of portions of your work control processes.

Continued strong management attention to these areas is warranted.

These violations are cited in the enclosed Notice of Violation and the circumstances n,urrounding the violations are described in detailin the enclosed report. Please note that you are required to respond to this letter and should follow the instructions specified in the enclosed Notice when preparing your response, in your response, you should document the specific actions taken and any additional actions you plan to prevent recurrence. Your response may reference or include previous docketed correspondence if the correspondence adequately ac' dresses the required response. The NRC will use your response, in part, to determine whether further enforcement action is necessary to ensure compliance with NRC regulatory requirements.

In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of this letter and its enclosures will be placed in the NRC Public Document Room (PDR). To the extent possible, your response should not include and personal privacy, proprietary, or safeguards information so that it can be placed in the PDR without redaction.
