Inservice Inspection (ISI) Un't 1 a.

General This is the first outage of the second period of the second ten-year plan.

(1) Lambert, MacGill, Thecas, Inc. (LMT), C0 HAM Inspection (CONAM),

ard Westinghouse Electric Corporation (W) performed the ISI in accordance with ASME Section,1980 Edition, Winter 1981 Addenda.

The Level II and III personnel UT were qualified at the EPRI NCE Center after September 10, 1985.

(2) All manual UT was perfom.d by LMT using the pulse-echo UT flaw detection instruments and various angles and MHZ transducers.

Also, on many welds the master / slave ultrasonic system was used.

(3) The eddy current examinations (ET) were performed by CONAM, and the results were evaluated by Westinghouse and CONAM. The ET covered 100% of all accessible tubing within steam generators (W) Nos.11 and 12.

Program end Procedures The NRC inspector reviewed the ISI program and procedures and found them to be acceptable. NSP made no specific request for relief from the ASME Code in perfcnning this ISI s utage, c.

No tube sleeving was perfomed this outage or before.

Observation of Work Activities The NRC inspector observed work and had discussions with personnel during the ISI activities. These observations included the perfornance of the following:

(1) Eddy current examination of several steam gene.ator tubes on steam generators No. 11 and No. 12.

(2) Master / slave ultrasonic system being used.

(3) Independent eddy :urrent examination data review by CONAM and Westinghouse.

(4) Ultrasonic examination of Yeld "E" in steam gener& tor No. 11.

Three teams were performing ultrasonic ext.minations at once, approximately 400" of weld.

(5) Magnetic particle examination of primary inlet and outlet manway bolts for steam generator No. 12.

(6) Liquid penetrant examination of weld #183 in the refueling water storage tank discharge line.

No violations or deviations were identified.

paragraph) at the conclusion of the inspection. The inspector summarized the scope and findings of the intpection noted in this report.
