on April 26, 1988 at the Diablo Canyon site.
Mr. Maneatis opened the meeting with a welcome and introduced the representatives of PGKE.
He followed by discussing significant issues facing the company such as the rate case currently in front of the California Public Utilities Commission and current staffing levels.
Mr. Maneatis indicated that although company wide staff dropped in 1987, staffing in Nuclear Power Generation increased by almost 5X.
Mr. Maneatis then turned the meeting over to Mr. Shiffer and Mr. Townsend.
Mr. Townsend and Mr. Shiffer discussed current plant status.
Unit I was in the 51st day of an 83 day refueling outage, originally scheduled for 63 days.
Significant schedule delays resulted from a cautious approach to mid-loop oper ation and the repair of the CRDM seal welds.
Unit 2 was at 100$
power with a 90Ã capacity factor for the cycle and a
75% lifetime capacity factor.
Their discussion then centered on the Performance Improvement Program progress, as detailed in a June 15, 1987 letter from the licensee to the
Regional Administrator.
Efforts to make management's expectations clear to plant staff include an incentive program; a
news letter describing error and safety statistics; installing computers accessible to all personnel, which incl'udes an integrated communications system; and getting supervisors into the work areas and the plant.
Nr. Townsend initiated discussion on the licensee's efforts in root cause evaluation.
Improvements as a result of Technical Review Group 'chairman training were noted.
In addition, revision to the reactor trip evaluation procedure to incorporate methods of analysis currently informally used were expected to be in place by mid-May.
Mr. Martin commented that it is of vital importance to learn from experience and although PGSE appears to be behind other licensees in the region, PGSE is headed in the right direction.
Nr. Nartin noted that it was his impression that the tough items in the performance improvement plan had been slow in completion.
Examples of this were setpoint control and management walkdowns.
He stated that the program included a number of good actions and recommended management maintain interest and strive for timely completion.
Nr. Townsend opened the discussion on performance indicators.
He noted that personnel errors were increasing and identified actions that were being taken to correct the problem.
Mr. Kirsch asked if the increase in errors could be attributed to the quality of procedures and the ability to have them easily revised.
Mr. Townsend indicated that this was not the problem or even a principal contributor, and noted that On-The-Spot-Change requests had increased in recent months.
Nr. Townsend noted that an extensive'program to upgrade plant procedures had almost been completed.
However, I&C has the greatest number of procedures requiring revision.
As a result, the licensee plans to increase the I8C staff by 18, including a number of procedure writers.
Nr. Townsend introduced the subject of maintenance backlog.
Mr. Martin expressed concern that in interviews with various managers there was a
lack of understanding of the size and shape of the backlog; i.e. the kind of backlog to be expected and the types and age of items currently open.
He suggested that the licensee should develop a better understanding of open maintenance items.
Mr. Moore described the role of the Nuclear Engineering and Construction Services organization with regard to plant operations, and several support tasks it had undertaken.
He also discussed the Configuration Management Task Force which has been established to ensure that the plant design is understood and incorporated into plant operations.
He noted that the task force had made proposed findings; however, the decision as to which actions would be acted upon was planned for mid-June.

Martin noted that the program appeared to be a good one and stated that the licensee had the advantage of the Design Verification Program and having been their own Architect/Engineer.
He noted that the region intended to do more inspection in the area of engineering.
Mr. Skidmore briefly discussed the recent use of NRC safety system functional inspection (SSFI) type techniques.
Mr. McLane briefly
discussed significant issues for the licensee under review by the Office of Nuclear Reactor Regulation.
Mr. Knighton, of NRR, acknowledged these issues.
In closing, Mr. Martin noted that in many cases a review of licensee performance prior to the close of a SALP period can be very helpful.
In the area of operations he noted that the licensee improved performance.
However, issues of personnel proceeding in the face of uncertainty; observations of Control Room decorum; operating instructions on information tags; and a continued high number of annunciators indicates that there is a continued need for management focus in operations.
In the area of maintenance, it was noted that a number of findings in the foreign material exclusion program indicates that management expectations are not reaching the worker level.
Mr. Martin noted that a focus would be placed on the quality of technical work, the continued efforts in root cause identification, and guality Assurance and guality Control.
Mr.
Shiffer and Mr. Townsend reiterated their concerns with the problems noted.
The meeting concluded at about 2:00 p.m.
