SUBJECT:

NRC AUGMENTED.INSPECTION TEAM FINDINGS INVOLVING LOSS OF OVERHEAD ANNUNCIATORS (Ref: NRC Inspection Report 50-272;311/92-81, dated February 4, 1993)

Our letter dated February 4, 1993, informed you of the findings of an NRC Augmented Inspection Team (AIT) that reviewed the circumstances surrounding a loss of the Overhead Annunciator (OHA) system in the Salem Unit 2 control room on December 13, 1992. While the AIT identified several proximate causes for the OHA system failure, it concluded that the root cause involved a Nuclear Control Operator (or Operators) failing to follow procedures when troubleshooting the Remote Control Work Station (RCWS) associated with OHA system operation and performance. The AIT concluded that there were no safety consequences due to the loss of the OHA system.

We noted that the root cause determinations and fact-gathering efforts of the AIT and PSE&G were hampered due to some implicated personnel failing to be completely forthright and candid relative to their involvement in this matter. Notwithstanding this difficulty, the AIT and PSE&G were still able to determine the causes of the OHA system failure sufficiently to identify and implement corrective measures to prevent recurrence. Upon completion of the AIT activities and evaluation of the matter by the NRC staff, specific personnel wrongdoing could not be substantiated, and no other enforcement action appeared warranted.

However, *we remain concerned about the apparent lack of candor demonstrated by one or more of your personnel. Though you addressed this matter in correspondence dated December 17, 1993, we believe that you need to examine. this concern further to determine if.there are improvements that can be made to your practices to assure that your staff willingly provides complete, prompt and accurate information about events and conditions that may occur. Further, -

you should assure that your staff, particularly licensed personnel, understand that information provided to the NRC, or to you as the licensee, whether verbal or written, is expected to be complete and accurate in all material respects; and that civil and criminal sanctions may be imposed against any individual who deliberately provides inaccurate or incomplete information.
