Licensee Action On LERs (Closed) LER 255/83-06-01: The licensee issued an event report upon discovery of discrepancies between the SIRW tank support structure and as-built documents during an A/E review of the support structur Fourteen three-foot square openings were present in the concrete beams versus nine openings identified on~the as-built documentation. See Paragraph 6 for additional detail . Licensee Action On IE Bulletins (Closed) IE: Bulletin (255/83-06-B8): IE Bulletin 83-06, " Nonconforming Materials Supplied By Tube-Line Corporation." The inspector reviewed the licensee's final response dated November 21, 1983. CPCo identified seventeen stainless steel fittings as being supplied by Tube-Line. Nine

of the fittings were returned to Tube-Line and eight were installed in the Auxiliary Feedwater Pump Suction Line (AF System), which is safety-related. The installed fittings met the requirements of the original installation code (ANSI B31.1 and ASME Code Case N-7) for replacements, and ASTM A-40 Minor discrepancies concerning the number of fittings and heat codes were noted by the inspector during the review of the licensees final response; however, these were resolved to the satisfaction of the inspecto . This is'the first outage'of the second interval. CPCo utilized the services of Consumers Power Laboratory .and Field Technical Services (L&FTS), Combustion Engineering (CE), and Westinghouse (W) to perform the ISI in accordance with ASME Section XI, 1977 Edition, Sumer 1978 Addend Eddy Current examination was performed on steam generators A and B, and the following tubes were plugged in steam generator B:

Safety Injection And Refueling Water (SIRW) Tank Support Structure Repair General This is a followup of inspections documented in Inspection Reports No. 50-255/83-08, No. 50-255/83-23 and No. 50-255/84-1 These inspections are as a result of LER 83-06, dated February 17, 1983, which documented a discrepancy between the actual configuration and the'as-built design documents for the SIRW tank support structur The SIRW tank cellular foundation slab is located over the Palisades plant control room. Each slab is one foot thick with a four foot space between them (see Figures 1 and 2). The calculated stresses in the foundation exceeded the allowable stresses when calculated in accordance with the Final Safety Analyses Report (FSAR) requirements and American Concrete Institute (ACI) Code 318-63. This' condition resulted from the foundation being constructed with several (14)

penetrations in the concrete beams that had not been accounted for in the original design calculation ,

The repair (see Figures 3 and 4) consisted of filling all eight

. penetrations in the north-south beams with reinforced aggregate-grout mix (Masterflow 713 Grout with Pea Gravel added). The east-west beams are secondary support members, which by analysis, have a negligible effect on the load carrying capacity of the slab system and are not included in the repair program. The repair will bring the stress levels of the foundation slab to within the allowable limits stipulated in the Palisades' FSAR and ACI 318-63 (concrete code) thereby resolving the stress problems in the structur CPCo Engineering and Field Operations (E&FO) was responsible for the repairs with Townsend and Bottom (T&B) performing the civil / structural wor Plan / Procedure Review
