Containment Building Tendon Surveillance, Units 3 and 4 The inspector examined procedures and quality records relating to the Units 3 and 4 tendon surveillance and work activities relating to tha Unit 4 tendon surveillance. Details of the inspection were as follows:

Review of Tendon Surveillance Procedure - Units 3 and 4 The inspector examined the following procedures which control the tendon surveillance activities.
(1) Procedure number 5177-187-CP-1, Tendon Surveillance Procedure for Containment Post-Tensioning System, Unit 3 L

(2) Procedure number 5177-187-CP-2, Ter. don Surveillance Procedure for Containment Post-Tensioning System, Unit 4 Acceptance criteria examined by the inspector appear in Technical Specification 4.4.5 and FSAR Section 5.1.7.4.

Observations of Tendon Surveillance Work Activities - Unit 4 The inspector witnessed the removal of a wire for inspection and testing from tendon number 3D28.
Removal and inspection of the wire was in accordance with the Unit 4 tendon surveillance procedure.
Samples were obtained from the wire and tagged for shipping to an off-site laboratory for tensile testing.
The inspector examined the anchorage assemblies on both ends of tendon 3D28 and verified that inspection of the anchorage assemblies was in accordance with the procedure.
The inspector witnessed retensioning of tendon 3028 and verified that the tendon was restressed to the force required by the procedure. The inspector measured the tendon elongation during reten-sioning and the new shim depth on the
"B" side of the tendon and veri-fied that these measurements coincided with the measurements recorded by the Bechtel QC inspector.

Review of Quality Records Relating to Tendon Surveillance Activities -
Units 3 and 4 The inspector examined -the following records relating to tendon surveillance.
(1) Surveillance inspection records for Unit 3 tendon numbers 3D28, 2028, 1D53, 42H70, 64H50, 64H51, 62HS, 45V7, 61V1, and 23V1.
These records included the results of end anchorage inspection, lif t-off data, and sheathing filler inspection.
(2) Surveillance inspection records for Unit 4 tendon number 64H70, 42H80, and 62H38. These records included the results of the end anchorage inspection and lift-off data.
(3) Stressing ram calibration records for ram numbers 4045005, 4045010, and 4045012.
No violations or deviations were identified.
