Inspection on January 30-February 1, 1979 (Report No. 50-219/79-02)

Areas Inspected:

Routine, unannounced inspection by regional based inspectors of general employee training, unlicensed personnel training, operator recualification training and licensee action on previous inspection findings.

The inspection involved 44 inspector hours on site by 2 NRC regional based inspecte s.

Results:

Of the five areas inspected, no items of noncompliance were identified.

Licensee Action on Previous Inspection Findings (Closed) Unresolved Item (%I9/78-03-01): A review of records showed that all Group Shift Supervisors received the requisite fire school trainin<j.

.(0 pen) Deficiency (219/78-03-02):

Licensee actions were consistent with the response to the item of noncompliance dated April 21, 1978.

The licensee identified that the records containing " Job Qualification Checksheets" were incomplete during reviews conducted on October 12, 1978, and January 16, 1979.

Since the " Job Qualification Checksheets" are also used as part of the personnel evaluation process, JCP&L requires that the checksheet be discussed with the employee by his supervisor and the employee's signature obtained on the checksheet.

In this process of review and evaluation, 11 of the records had been misplaced. The licensee had identified the missing records, and at the close of this inspection, was preparing replacements for the mining records.

The subject records will be inspected during a future inspection to verify that full compliance is achieve.

3.

General Employee Training a.

Program Definition The inspector reviewed the licensee's procedures with respect to the program definition requirements of:

10 CFR 50, Appendix B, Criterion II; 10 CFR 19.12; 10 CFR 73.50; and ANSI N18.1.

The procedures set fcrth formal training programs for:

new and existing employees; temporary employees; technicians; and, craft personnel.

These programs establish training which covers: administrative controls and procedures; radiological health and safety; controlled access and security; industrial safety; emergency plans and procedures; and quality assurance indoctrination.

Formal training is also provided for female employees on the contents of Appendix A to Regulatory Guide 8.13.

b.

Program Participation The inspector reviewed the licensee's records to assure that the required training had been given.

In addition, the inspector conducted interviews with certain of those individuals whose records were reviewed.

The interviews verified that:

the scope of the training was similar to that contained in the licensee's records; the training as conducted was meaningful to those attending; and that the areas presented were covered accurately and sufficiently from the participants' point of view.

Records were reviewed for and interviews conducted with employees as listed below:

two relatively new employees;

--

two employees with more than ene year of service; and

--

one temporary employee.

--

No items of noncompliance were identified regarding general employee trainin.

4.

Craft Personnel Training a.

Program Definition The inspector reviewed the training procedures and require-ments for craft and technician training as defined in the

"0yster Creek Training Administration Manual", Revision 5.

The inspector determined that the Manual provided for on-the-job and formal technical training for initial and continuing training. These requirements are applicable to mechanical, electrical, instrument and control, and chemistry department craft personnel and technicians.

No items of noncompliance were identified.

b.

Program Implementation The inspector reviewed the licensee's training activities in regard to implementation of the program defined by the above Manual.

The station is providing the following types of training for personnel noted in Paragraph 4.a:

in-plant lectures of a general information nature;

in-plant courses specifically applicable to mechanical

--

crafts and chemistry technicians; contractor presented courses for instrument and control

--

and electrical crafts; in-plant " current awareness" program which routes new/re-

--

vised procedures, LER type information, etc. to individ-uals as required reading; and,

--

both initial and continuing on-the-job training specific to the individual's job classification.

At the tima of this inspection, the licensee had identified the need for and was formulating plans for upgrading E0 training but had not yet issued appropriate training procedures for the administration of an E0 replacement training and retraining program.

The licensee stated that a program, meeting the requirements of ANSI N18.1-1977, will be established by an administrative procedure issued and implemented by August 1, 1979. This item is unresolved pending NRC:RI review of the subject procedure and its implementation (219/79-02-02).

Discussions with licerisee personnel regarding the training and qualifications of quality assurance staff members revealed that the Quality Assurance Department's internal training program was administered and the records kept by quality assurance staff at the corporate offices.

Since the records and programs were not available on site, they will be inspected during a future insnaction at the corporate offices.

This item is unresolved (219/78-02-04) pending completion of the above inspection.

No licensee action is required.

5.

Requalification Training a.

Program Review The inspector reviewed the licensee's program with regard to the requirements set forth in the accepted Operator Requali-fication Program and verified that, as currently established, the program includes the following:

an established, planned, continuing lecture schedule

--

appropriate to deficient areas identified by the most recent annual exams; documentation of personnel attendance;

--

required reactivity control manipulations;

--

discussions / reviews of changes in facility design, pro-

--

cedures, ar facility license; and, review of abnormal / emergency procedures.

--

The inspectors determined that no changes have been made to the program which are not in conformance with existing regulatory requirements or comnitments.

No items c' noncompliance were identified.

b.

Record Review The inspector selected and reviewed the records of five licensed operators to verify that each contained the followiia documenta-tion:

completed course and yearly examinations with answers;

--

manipulation of controls for reactivity changes required

--

by the program; simulation or discussion of emergency / abnormal procedures

--

and responses; results of supervisory performance evaluations; and,

--

attendance or makeup for required lectures.

--

The inspectors determined by interview and record review that, since the last inspection of the program by NRC:RI, no licensed operators had:

failed all or portions of the annual exam;

--

received unsatisfactory performance evaluations; or,

--

did not perform licensed duties for a period of four

--

months or longer.

Individual records were reviewed for two licensed Control Room Operators, two licensed Group Shift Supervisors and one licensed staff membe.

During review of general requalification program records, the inspectors noted that seven operators had not submitted the completed SF 405 Emergency Procedure Review acknowledgement forms for the training cycle completed on December 17, 1978.

These discrepancies had been identified by a training staff records review prior to the inspection and corrective action had been initiated. At completion of this inspection, only one form had not yet been submitted. This individual was off duty during the period. All forms inspected indicated that the Emergency Procedure reviews had been accomplished as required.

Inspector review of the 1978 annual examination results for the subject individuals indicated satisfactory perforunce in each examination subject area concerning emergency procedures.

No items of noncompliance were identified.

c.

Personnel Interviews The inspector selected and interviewed three licensed personnel.

The interviews were directed at obtaining, from these individuals, a subjective appraisal of the content and effectiveness of the requalification training as presented and to confirm that the training administered was similar to that contained in the licensee's records.

The inspector identified no discrepancies or inconsistencies between the interview results and the licensee records.

No items of noncompliance were identified.

6.

Inspection, Examination and Testing Personnel Station Procedure No. 4012, " Operational Quality Assurance Personnel Qualification and Certification" provides requirements for certifying station and/or contractor personnel who perform inspection, examination or testing activities to the requirements of ANSI N45.2.6.

The inspector reviewed the ANSI N45.2.6 certification records for seven contractor personnel who are authorized to perform such activities at the Oyster Creek Station.

The inspector identified no discrepancies in documentation with the requirements of the above procedure.

7.

Review / Audit Function of Training Coordinator The Oyster Creek Training Administration Manual provides for informal audits, conducted by the Training Coordinator, of station training activities and related records, including those of other station departments.

Discussions with station management indicated that these " audits" were intended to be management overviews conduc.ed to identify problems or potential problems to management allowing immediate and expedient correction.

The audits are not intended to be formalized to the extent defined in the licensee's Quality Assurance Plan.

The inspectors, however, observed that the Training Administration Manual provides no guidance for documentation, notification of responsible management or dispostion of problems or discrepancies identified during these reviews / audits.

Licensee representatives agreed that this function should be better defined and corrective actions documented in that the reviews are utilized to monitor actions taken to correct NRC inspection findings as well as internally identified problems.

This item is unresolved pending licensee revision of the Training Administration Manual pursuant to the above (219/79-02-03). The licensee stated that this revision would be incorporated on a routine basis in conjunction with other upcoming revisions.

8.

Unresolved Items Unresolved items are matters about which more information is re-quired in order to ascertain whether they are acceptable items, items of noncompliance or deviations.

Unresolved items disclosed during the inspection are discussed in Paragraphs 4 and 7.

9.

Exit Interview The inspector met with licensee representatives (denoted in Para-graph 1) on Feburary 1, 1979, at the Oyster Creek Station.
