Subject: NOTICE OF VIOLATION (Inspection No. 50-213/88-12)

This letter refers to the special safeguards inspection conducted on May 17-20 and May 26 - June 1,1988 at the 'iaddam Neck Nuclear Power Plant, Haddam Neck, Connecticut, to review the circumstances associated with two examples of an apparent violation of the NRC-app, oved physical securits plan at Haddam Nec The inspection report was sent to ,vou on June 24, 1988. Both examples, which involved degradations of vital area (VA) barriers, were identified by your staff. On July 6,1988, an enforcement conference was conducted with Mr. W. Romberg and other members of your staff to discuss both examples, their causes, as well as your corrective action The violation, which is described in the enclosed Notice of Violation, involved:

(1) an unlocked and unmonitored opening in a temporary extension of a vital area barrier, for approximately 46 minutes during the recent outage in May 1988; and (2) a 13.5 in. by 8 in. opening in another vital area barrier, for app oxin;ately two days in May 1985. In each case, until the degradations were identified by security personnel, compensatory measures were not take In the first case, a security shift. supervisor performed a walkdown of the temporary extension prior to the vital area barrier being extended, without recognizing the unlocked door in the barrier extension. The individual did not have a procedure specifying the openings in the barrier extension which are required to be verified as secured during the walkdown. In the second case, the hole in the vital area barrier was created by construction persenn91 as part of a plant modification. However, :ecurity personnel were not contacted as required prior to implementation of this portion of the modification. These events demonstrate the importance of adequate procedures for performing activities, and adequate coordination with the security department whenever changes are made at the facilit Both of these barrier degradations involved the loss of two security elements, namely, barrier and monitoring. After reviewing your response to this Notice, including your proposed corrective actions, the NRC will determine whether further NRC enforcement action is necessary to ensure compliance with i NRC regulatory requirement .
