A License Condition 15A of Byproduct Material License 20-06216-01 requires that each sealed source containing byproduct material, other than Hydrogen-3, with a half life greater than thirt;

days and in any form other than gas shall be tested for leakage and/or contamination at intervals not to exceed six months.

Contrary-to this reouirement, four Co-60 sources of 1 millicurie, 1.71 millieuries, 35.19 millicuries and 115.9 millicuries; and one Cs-137 source of 50 curies were not tested between June 4,

1975 and February 1,1976, an interval of eight months.

Status of Previous 1v Tdentified Unresolved Items

Inspection Report 75-09, Detail 5.e. reported that the licensee had

adopted and utilized a procedure for sampling prior to vapor contain-ment entry,-before the procedure had been formally reviewed and accepted.

In the course of this inspection, the inspector noted that the pro-cedure has been formally approved by the Plant Offsite Review Committee (PORC). The inspector had no further questions on this matter.

Enforcement Action a.

Sealed Source Leak Yest j

In reviewing the licensee's record of sealed source leak testing, the inspector noted one case of an eight month period between leak test intervals, i.e. June 4, 1975 to February 2, 1976. This

interval is contrary to the License condition 15.A of License 20-06216-01, which specifies that the interval between leak testing shall not exceed six months.
