Dear Commissioners and Staff:

Pursuant to 10 CFR 26, Appendix A, Subpart B, Section 2.8 (e)(4), Pacific Gas and Electric Company (PG&E) is submitting the following report of an investigation into an unsatisfactory performance test result associated with the Diablo Canyon Power Plant (DCPP) Fitness For Duty (FFD) Program.

On August 11, 2008, a blind performance specimen processed at the Laboratory Corporation of America (LabCorp) testing facility was reported to the DCPP FFD Program Supervisor with a negative result. The performance sample had been spiked with phencyclidine (PCP), and a positive result should have been reported.

LabCorp was contacted regarding this false negative result. LabCorp provided a report which indicated that: "[d]uring the review and posting of results from the 2 batches, the PCP result for this specimen was inadvertently entered as negative instead of 40 ng/ml. A corrected report was issued on August 13, 2008 documenting the change from negative to positive at 40 ng/ml." Enclosed with this submittal is the report from LabCorp. The LabCorp report also indicates: "This-issue has been reviewed with the certifying scientist involved and training will be conducted with all certifying scientists regarding accurate entry of GC/MS results."

PG&E makes no regulatory commitments under this submittal.

Dear Mr. Ryan,

I received correspondence indicating that the result reported to you on a Blind Quality Control specimen was not consistent with the result you expected on the Blind QC sample per your manufacturer's Certificate of Analysis. Laboratory Corporation of America at Research Triangle Park, North Carolina reported P13559 (Lab Accession Number 0314705613) as negative yet the expected result was positive for PCP.

This specimen did yield a presumptive positive result on the initial test for PCP. It was extracted and analyzed by GC/MS in 2 different batches. During the review and posting of results from the 2 batches, the PCP result for this specimen was inadvertently entered as negative instead of 40 ng/mL. A corrected report was issued on August 13, 2008 documenting the change from negative to positive at 40 ng/mL.

This issue has been reviewed with the certifying scientist involved and training will be conducted with all certifying scientists regarding accurate entry of GU/MS results.
