Title | Failure to Provide Procedure Instructions Results in Exceeding Technical Specification Heat Up Rate During Plant Start Up |
Description | A Green finding and an associated non-citied violation of Technical Specification 5.4, Procedures, were self-revealed when the licensee failed to establish instructions in station procedure CPS 9059.01, Reactor Coolant System Leakage Test, Revision 9b. Specifically, the licensee failed to provide instructions to ensure that the main steam piping between the reactor vessel and the inboard main steam isolation valves were completely drained of water at the completion of testing. The licensee entered this issue into the corrective action program as AR 01590671 and provided operations personnel involved in startups just in time training. The inspectors determined that the licensees failure to establish instructions to ensure that the main steam piping between the reactor vessel and the inboard main steam isolation valves were completely drained of water prior to starting up the reactor was a performance deficiency. accordance with IMC 0612, "Power Reactor Inspection Reports," Appendix B, "Issue Screening," dated September 7, 2012, because, it was associated with the equipment performance attribute of the mitigating systems cornerstone and adversely affected the cornerstone objective of ensuring the availability, reliability, and capability of systems that respond to initiating events to prevent undesirable consequences, and is therefore a finding. The finding was screened against the Mitigating Systems cornerstone and determined to be of very low safety significance because the finding was/did not: (1) a deficiency affecting the design or qualification of a mitigating structure, system or component, (2) represent a loss of system and/or function, (3) represent an actual loss of function of a single train for greater than its technical specification allowed outage time, (4) represent an actual loss of function of one or more non-technical specification trains of equipment designated as high safety-significant for greater than 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br /> and (5) did not involve the loss or degradation of equipment or function specifically designed to mitigate a seismic, flooding or severe weather event. The inspectors determined this finding affected the cross-cutting area of human performance in the aspect of work management where the organization implements a process of planning, controlling, and executing work activities such that nuclear safety is the overriding priority.
