| Description | 10 CFR Part 50, Appendix B, Criterion V, Instructions, Procedures, and Drawings, requires, in part, that activities affective quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedure, or drawings. Contrary to the above, since the original licensing of the plant on May 20, 1988, licensee personnel failed to prescribe an adequate procedure for performing a surveillance stroke test for the Unit 2 CS valve 2CS009B since this condition was not analyzed assuming an active single failure for this valve to close. This finding was determined to be of very low safety significance based on an estimated core damage probability of 9.6 E-8. This issue was entered into the stations CAP that prescribed a cause evaluation, extent of condition review, and actuator rebuild. |
