CONDITIONS PRIOR TO EVENT

Unit 1 — Mode 1, approximately 10 percent Rated Thermal Power Unit 2 — Mode 1, approximately 100 percent Rated Thermal Power There were no structures, systems, or components that were inoperable at the start of the event that contributed to the event.

EVENT DESCRIPTION

On April 22, 2016 at approximately 11:30, a Nuclear Plant Operator (NPO) was entering a Unit 1 Reactor Building [EMS System Identifier: NG] access door [EIIS Component Identifier: DR] and noticed another worker entering the airlock door behind him with a cart. The NPO stated that he was trying to be helpful and opened the reactor building side (inner) door. The NPO stated that they did not notice that the turbine building side (outer) door was still open. Both doors were open at the same time for approximately one second. This airlock serves as a secondary containment boundary [El IS System Identifier: NG]; as such, having both doors open at the same time results in failure to meet Technical Specification 3.6.4.1 as a result of not satisfying Surveillance Requirement 3.6.4.1.3.

The condition requires a Licensee Event Report (LER) in accordance with 10 CFR 50.73(a)(2)(v)(C).

CAUSE OF EVENT

The direct cause of the event was less than adequate verification practices to ensure compliance with Secondary Containment door requirements.

ANALYSIS/SAFETY SIGNIFICANCE

An engineering evaluation was performed and concluded that secondary containment could have performed its safety function of isolating, as assumed in the accident analysis, and also of re-establishing 0.25 inches vacuum (drawdown) within the assumed accident analysis time (10 minutes). Therefore, the subject event did not cause a loss of safety function. This event will not be counted as a safety system functional failure (SSFF) for the NRC performance indicator based on the engineering analysis that shows there was no loss of ability to fulfill the safety function.

CORRECTIVE ACTIONS

Corrective actions include holding the individual accountable for less than adequate verification practices.
