On Monday, January 30, 1995, Chemistry Department personnel discovered that a procedure which had been revised via a temporary on-the-spot change (OTSC) on December 23, 1994, had not been reviewed and approved within 14 days as required by Technical Specification 6.8.3.

The procedure had been temporarily modified to accommodate operation of the filter backwash system with a failed pressure control valve.

A temporary OTSC was prepared as it was expected that the pressure control valve would be replaced by the end of January.

The procedure had been used on a weekly basis since the OTSC was approved on 12/23/94.

A root cause investigation of this event determined that the event was due to improper work practices.

The Chemistry Department procedure coordinator, who is responsible for updating procedures and initiating the review, did not implement the review process specified in Nuclear Department Administrative Procedures which govern the processing of on-the-spot changes.

The individual involved in this event has been counseled with regard to proper work practices.

Reactor Power 95% of rated, 1050 MWe DESCRIPTION OF OCCURRENCE On Monday, January 30, 1995, Chemistry Department personnel discovered that a procedure which had been revised via a temporary on-the-spot change (OTSC) on December 23, 1994, had not been reviewed and approved within 14 days as required by Technical Specification 6.8.3.

The procedure had been temporarily modified to accommodate operation of the filter backwash system with a failed pressure control valve.

A temporary OTSC was prepared as it was expected that the pressure control valve would be replaced by the end of January.

The procedure had been used on a weekly basis since the OTSC was approved on 12/23/94.

ANALYSIS OF OCCURRENCE A root cause investigation of this event determined that the event was due to improper work practices.

The Chemistry Department procedure coordinator, who is responsible for updating procedures and initiating the review, did not implement the review process specified in Nuclear Department Administrative Procedures which govern the processing of on-the-spot changes.

The individual was aware of the OTSC being implemented as he received a copy through the normal distribution process, prior to the expiration of the 14 day review period.

In cddition, a weekly report identified the procedure as having an open OTSC, but because it was a temporary OTSC he did not believe it required the 14 day review.

The individual did not reference the Administrative Procedure until the engineer, who had prepared the OTSC, identified that the procedure did require the review regardless of the OTSC being temporary or permanent.

SAFETY SIGNIFICANCE

This event posed minimal safety significance.

Although the review was not performed when required, the changes were determined to be cppropriate and approved without modification.

PREVIOUS OCCURRENCES

j There has been no previous similar event reported.

CORRECTIVE ACTIONS

i The individual involved in this event has been counseled with regard to proper work practices.

This event will be reviewed with other department procedure coordinators to ensure a similar misconception regarding temporary on-the-spot changes does not exist.
