In accordance with the Trojan Plant Operating License, Appendix A USNRC Technical Specification 6.9.1.9.c, attached is Licensee Event Report No.

81-32 concerning a failure to document the verification of auxiliary feed-water diesel day tank level at the required frequency. This was an inadequacy in the implementation of administrative controls which threatened to reduce the degree of redundancy required by the Technical Specifications, paragraph 3. 7.1. 2.

Identification of Occurrence:

During a QA audit it was determined that auxiliary feedwater diesel Jay tank level verifiction had been deleted in a revision to a Periodic Operating Test (POT-5-1) which was issued on January 13, 1981. Theref ore, no written documentation e: cists to show compliance with Technical Specification 4.7.1.2.2.a.1.

Conditions Prior to Occurrence:

The plant was in Mode 1, power operations at 100% power when the omission was discovered.

Description of occurrence:

To accommodate new inservice pump and valve testing (IST) requirements POT-5-1 was extensively rewritten.

The day tank level verification was inadvercently omitted f rom the new revision. Although the day tank is alarmed and has an automatic makeup system no written documen-tation exists to show monthly verification of the fuel 1cvel.

Designation of Apparent Cause of Occurrence:

The occurrence was attributed to administrative error namely perfor-mance of an inadequate technical review of the procedure revision.

There was no effect on either plant or public safety since the day tank level is alarmed and has an automatic makeup system, i

10. Corrective Action

A new revision to the POT has been written which cantains a verifica-tion sign-off for diesel fuel-oil day tank level. Additionally this specific problem has been discussed with Plant Engineers as an example of the importance of a thorough technical review of changes to saf ety-relateu procedures.

Finally it was determined the existing procedure for performance of technical reviews of safety-related procedures was adequate.
