Facility: Trojan Nuclear Plant, PO Box 439, Rainier, OR 97048 4.

Identification of Occurrence:

During routine operations surveillance, it was discovered that MD-059, the manual containment isolation valve for demineralized water wash-i down was in the open position.

Conditions Prior to Occurrence:

Prior to the occurrence the plant was in Mode 1 at 80% power.

Description of Occurrence:

While performing routine surveillance checks of locked valves, MD-059, i

the manual containment isolation valve for demineralized water wash-i-

down was found in the open position.

7.

Designation of Apparent Cause of Occurrence:

l MD-059 was opened on July 15, 1981 to provide a water supply for troubleshooting containment sump level probes. After use the valve was apparently left open.

8.

Significance of Occurrence:

This event had no effect on plant or public safety since the redundant i

l containment isolation valve was operable.

9.

Corrective Action

1 Corrective action was to shut and lock the valve and verify its i

posicion by two independent checks. Further action was taken to i,

verify that all manual containment isolation valves are included in the locked valve list.
