Containment Fan Coil Unit - Service Water Leak.

This report was initiated by Incident Reports81-469 and 81-470.

CONDITIONS PRIOR TO OCCURRENCE:

Mode 1 - Rx Power 97% - Unit Load 1100 MWe.

DESCRIPTION OF OCCURRENCE:

On November 19, 1981, while performing OI II 1.3.5, Reactor Coolant Leak Detection, it was determined that there was approximately 1.1 GPM unidentified leakage.

Technical Specification 3.4.7.2.b specifies a limit of 1 GPM.

Action Statement 3.4.7.2.b was entered at 0915 hours0.0106 days <br />0.254 hours <br />0.00151 weeks <br />3.481575e-4 months <br />.

At 1115 hours0.0129 days <br />0.31 hours <br />0.00184 weeks <br />4.242575e-4 months <br /> a containment inspection was made and the operator determined that approximately 0.5 GPM of service water was leaking from the No. 22 Containment Fan Coil Unit (CFCU)

Motor Cooler.

Action Statement 3.4.7.2.b was terminated at 1115 hours0.0129 days <br />0.31 hours <br />0.00184 weeks <br />4.242575e-4 months <br />.

No. 22 CFCU was isolated and Action Statement 3.6.2.3.a was entered at 1115 hours0.0129 days <br />0.31 hours <br />0.00184 weeks <br />4.242575e-4 months <br />.

This occurrence constituted operation in a degraded mode in accordance with Technical Specification 6.9.1.8.c.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

The service water leak was due to a pinhole leak in No. 22 CFCU Motor Cooler.

Technical Specification 3.4.7.2.b requires:

With any Reactor Coolant System unidentified leakage greater than 1 GPM, reduce the unidentified leakage rate to within limits within 4 hours4.62963e-5 days <br />0.00111 hours <br />6.613757e-6 weeks <br />1.522e-6 months <br /> or be in at least hot standby within the next 6 hours6.944444e-5 days <br />0.00167 hours <br />9.920635e-6 weeks <br />2.283e-6 months <br /> and in cold shut-down within the following 30 hours3.472222e-4 days <br />0.00833 hours <br />4.960317e-5 weeks <br />1.1415e-5 months <br />.

Technical Specification 3.6.2.3.a requires:

With one group of containment cooling fans inoperable and both Containment Spray Systems cperable, restore the inoperable cooling fans to operable status within 7 days or be in at least hot standby within the next 6 hours6.944444e-5 days <br />0.00167 hours <br />9.920635e-6 weeks <br />2.283e-6 months <br /> and in cold shutdown within the following 30 hours3.472222e-4 days <br />0.00833 hours <br />4.960317e-5 weeks <br />1.1415e-5 months <br />.

CORRECTIVE ACTION

After the action statement was entered, the reactor tripped, and in order to return the unit to operation, Action Statement 3.6.2.3.a was terminated at 1836 hours0.0213 days <br />0.51 hours <br />0.00304 weeks <br />6.98598e-4 months <br />, as required by appli-cability statement 3.0.4.

Subsequently, No. 22 CFCU Motor Cooler was replaced, tested satisfactorily, and returned to service.
