| Description | On February 27, 2015, MSSV 46-2, 45-4, and 47-4 failed to meet the TS as-found lift setpoint test. Additionally, on March 22, 2015, MSSV 46-3 failed to meet the TS asfound lift setpoint test. TS Limiting Condition for Operation 3.7.1.A, Main Steam Safety Valves, requires if one or more required MSSVs are inoperable, reduce neutron flux trip setpoint to less than or equal to the applicable percent Reactor Thermal Power listed in Table 3.7.1- 1 within 4 hours4.62963e-5 days <br />0.00111 hours <br />6.613757e-6 weeks <br />1.522e-6 months <br />. If the required action and associated completion time is not met, the reactor shall be placed in Mode 3 in 6-hours and Mode 4 in 12 hours1.388889e-4 days <br />0.00333 hours <br />1.984127e-5 weeks <br />4.566e-6 months <br />. Contrary to this requirement, MSSV 46-2, 45-4, 47-4, and 46-3 were inoperable for a time period that exceeded the TS allowed outage. This finding was determined to be of very low safety significance (Green) because the finding did not represent a loss of safety function for the MSSV system. This issue was documented in Entergys CAP (CR-IP3-2015-0898 and CR-IP3-2015-2128) and reports were made to the NRC in LER 05000286/2015-002-00 and 2015-002-01. |
