Joseph M. Farley Nuclear Plant - Unit 2 Special Report 2018-002*00, Nonfunctional Radiation Monitor R-298 Enclosure Special Report 2018-002*00

Enclosure 01 to NL-18-1391 Special Report 2018-002-00 Nonfunctional Radiation Monitor R-298 This Special Report is being submitted due to the Unit 2 Plant Vent Stack Noble Gas Activity Monitor (R-298) not being restored to functional status within the time required by Technical Requirement (TR) 13.3.4 of the Joseph M. Farley Nuclear Plant (FNP) Technical Requirements Manual (TAM). The TR requires that the monitor be restored to functional status within 7 days or submit a Special Report within the following 14 days.

This report outlines the requirements of TR 13.3.4 Condition 0, Action 0.1 to prepare and submit a report outlining the action taken, the cause of the non-functionality and the plans and schedule for restoring the system to FUNCTIONAL status.

These actions will also be submitted in the next Radioactive Effluent Release Report per the Farley Offsite Dose Calculation Manual (OOCM).

Summarv of the Instrument Loss of Function:

At 0945 on October 23, 2018, with Unit 2 in Mode 1, Radiation Monitor R-298 was declared NON-FUNCTIONAL as part of routine maintenance to perform calibration in accordance with plant procedures. The TR Required Action was entered for R-298 and compensatory measures were established.

Pre-planned Alternate Method of Monitoring:

R-29C (Plant Vent Stack Particulate, Iodine, and Noble Gas Monitor) has remained FUNCTIONAL.

In the event both Radiation Monitors (R-298 and R-29C) became NON-FUNCTIONAL, the actions of the OOCM (Action 37) would be required. With less than the required minimum channels OPERABLE, effluent releases via this pathway may continue provided grab samples are taken at least once per 8 hours9.259259e-5 days <br />0.00222 hours <br />1.322751e-5 weeks <br />3.044e-6 months <br /> and these samples are analyzed for gross activity within 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br />.

Cause of the Loss of FUNCTION:

The initial cause of the loss of function was planned removal from service for calibration. During the calibration of R-298, a deficiency was identified that prevents the low range pump from starting.

Plans and Schedule for Restoring the Instrumentation Channel to FUNCTIONAL status:

Replacement parts were expedited and received on November 10, 2018, and calibration is expected to be completed on or before November 16, 2018.
