Joseph M. Farley Nuclear Plant, Unit 2 Special Report 2016-003-00, Nonfunctional Radiation Monitor R-60A Enclosure Special Report 2016-003-00 This Special Report is being submitted due to the Unit 2 radiation monitor R-60A (Main Steam Relief and Atmospheric Steam Dump Discharge) not being restored to functional status within the time required by Technical Requirements Manual (TRM) TR 13.3.4. The TRM requires that the monitor be restored to functional status within 7 days or submit a Special Report within the following 14 days.

At 0225 on June 17, 2016, with Unit 2 in Mode 1, radiation monitor R-60A was declared nonfunctional due to an error in the display. A mandatory TR Required Action was entered and a work order was generated to repair R-60A.

Troubleshooting of the error in the display isolated the problem to the detector.

Due to the detector's location on the main steam valve room roof being in close proximity to steam generator safety relief valve exhaust piping, repair of the monitor is a personnel safety concern should any relief valve lift during the repair effort. Therefore, repair of the monitor should be performed only under cold shutdown conditions to ensure personnel safety. A work order has been developed to repair the radiation monitor in the event of an unscheduled outage or the next refueling outage. In addition, a design change is scheduled to replace R-60A with a new radiation monitor. Compensatory measures have been established and TR Required Actions are being met for radiation monitor R-60A nonfunctionality.
