TVA letter to NRC, Reactor and Senior Reactor Operator Initial Examinations, dated September 19, 2011 The purpose of this letter is to submit approved Personal Qualification Statements (NRC Form 398) and Certifications of Medical Examination (NRC Form 396) for two reactor operator and three senior reactor operator license applicants for Watts Bar Nuclear Plant. TVA requests that the NRC administer the written examinations and operating tests to these individuals as discussed in Reference 1. Currently, the examinations are scheduled to begin starting December 19, 2011. TVA previously provided preliminary Personal Qualification Statements and Certifications of Medical Examination for the applicants in Reference 2.

U.S. Nuclear Regulatory Commission Page 2 Enclosure 1 provides the name, type of license being applied for, and the docket number for each applicant. Enclosure 2 provides the original set of the Personal Qualification Statements. Enclosures 3 and 4 provide the required two copies of the Personal Qualification Statements. The original Certifications of Medical Examination are provided in Enclosure 5.

This letter contains no regulatory commitments. This letter does provide personally identifiable medical information and is considered by WA to be a personnel and medical file. Therefore, WA requests that this information be withheld from public disclosure in accordance with 10 CFR 2.390(a)(6).
