Description

Technical Specification Limiting Condition for Operation (LCO) 3.6.9, Isolation Valve Seal Water System (IVSWS), requires the IVSWS to be operable in Modes 1, 2, 3 and 4. The required action for Condition A, one IVSWS header inoperable or one IVSW automatic actuation valve inoperable, is to restore IVSWS to operable within a completion time of seven days. If the IVSWS is not restored to operable within the completion time, Condition C requires the unit to be placed in Mode 3 within three hours and Mode 5 within 36 hours4.166667e-4 days <br />0.01 hours <br />5.952381e-5 weeks <br />1.3698e-5 months <br />. Contrary to this requirement, one IVSWS header was inoperable from April 5, 2011 until November 12, 2011 because IVSWS valve IV-1400 was out of position, as identified by an operators discovery during normal operator rounds that IV-1400, a normally open valve, was shut. Entergy personnel opened the valve and documented the issue in their corrective action program as CR-IP3-2011-05110. The inspectors assessed the issue in accordance with IMC 0609 Appendix A, The Significance Determination Process for Findings at Power, and determined this violation is of very low safety significance (Green) because the finding did not represent an actual open pathway in the physical integrity of reactor containment, containment isolation system or heat removal components.
