Secondarylidswith 10 numbers8-120B-5,-6,-7and-8donotcomplywithapprovedSafety AnalysisReport(SAR) drawings referencedinthe CoCo Specifically, the.secondarylidstestportshaveacladdingtubeinstalledwhichisnotreflectedintheCoC configuration.

Reference1isareportprovidedbythe certificate holder describingthiscondition.

Grand Gulf NuclearStation(GGNS) identified six shipments,madein2015,thatutilizedthe120B-7model secondary lids: 2015-0707, 2015-0801,2015-0804,2015-0805,2015-0809 and 2015-0901.

Energy Solutions suspendedtheuseofthe 8-120Bcaskswith secondarylidsandwillretrofitthem.Further,EnergySolutions,on September16,2015, submittedaCoC amendment requesttothe Nuclear Regulatory Commission (NRC)requesting authorizeduseofthesubject secondarylids.Energy Solutions maintainsthatthe presenceofthetubesinthetestportshasnosafety consequencebecausethe as-built test volumemeetsthebasisforthe pre-shipmentleakratetestholdtime.Thislettercontainsnonew commitments.Ifyouhaveany questionsorrequireadditionalinformation,pleasecontactMr.RoyMiller, Radiation ProtectionManager,at 601-437-7397.

10CFR71.95 notification associatedwiththefailureto observe Certificate of Compliance condition ofthe8-120B secondarylidtestport configuration.DearValued Customer:DuringarecentNRCaudit performed September8-10,2015 assessing implementation of EnergySolutions' 10CFR71 programatour Columbia,SCand Barnwell,SCfacilities,theNRCinspectionteam identifiedthatfourout ofeight8-120B secondary lids (Subject Secondary Lid TD numbers 8-120B-5,-6,-7and-8)didnotcomplywith approved 8-120B SAR drawings referencedinthe current revision of the Certificate of Compliance9168(CoC).Thesubject secondarylidswere fabricatedwitha claddingtubeinsidethetestporthole,butthetubeisnotshownontheSAR licensing drawings referencedbytheCoC.Basedonthis, the conditions ofapprovalintheCoCwerenot observedforanyTypeB shipments of radioactivematerialmadein8-120B casks utilizing the subject secondarylids.Subject secondarylidswere entered into service between October-December,2014andhavebeenusedforatotal of 52 shipmentsbylicensees.The condition was determinednottohavea safety consequencebecausethe as-built test volumeswithtubesinstalledin subject secondarylidsmetthebasisforthe pre-shipmentleakratetestholdtime.

Although, claddingtubeswerepart oftheoriginalNRC approved cask configurationintheold8-120B secondarylids(usedbefore August 31,2013),theywere inadvertentlyomittedinthenew secondarylidsdesignenteredinto service in September,2013.Duetothis condition, EnergySolutions suspendedtheuse ofthe8-120B casks with subject secondarylids.Further, EnergySolutions, on September16,2015, submittedaCoC amendment requesttoNRC requesting authorized usage of the subject secondary lids.EnergySolutionswillretrofitall 120B secondarylidswiththetubedesignatthe earliest opportunity.

We expectthattheNRCwillgrantanewCoC (Rev.22)shortlyandtheuse of the affected 8-120B caskswithsubject secondarylidswillresume.Allour8-120Buserswillbe providedwitha separate notificationandanupdated 8-120Bcaskbookwhichwi IIclarifythestatus of the affectedCoCthatthe120Bcaskscan operate under.Pleasesee EnergySolutions' attached report containing the informationrequiredby10CFR71.95.Inthereport, EnergySolutions describesthecause of the occurrenceandprovides information that supportsthatthere isnosafety significance associatedwiththis condition.

The purpose of the secondarylidtestporttubeistoisolatethe pre-shipment test volume from the interspace betweenthetwo31/4" thick platesthatmakeupthe secondarylid.The isolation ofthetest volume providedbythetube assuresthatthetest volumedoesnotexceedthelimitonwhichtheholdtimeforthe pre-shipmentleakratetestisbased.

However,ifthefreevolumeinthe interspace between the plates is negligible,thentheisolationfunction ofthetubeisnot necessary.Thetubewasshownasadesignfeatureinthe8-120BSAR drawings referenced by Revision 17 oftheCoC,and previous revisions,butthetubewasnot creditedwithanysafetyfunctionintheSAR.Thetubewas inadvertently omittedwhenthelidswere redesigned for operational enhancementsinCoC Revision19.The omission was identified by EnergySolutions' correctiveactionsystemwhilethe subjectlidswerestillinfabrication.A correctiveactionwas assigned to measure the as-built test volumes,whichincludedthevoid space betweenthethick platessincetherewerenotubesinstalled.Thetest volumes werefoundto exceedthevolumebasisfor pre-shipmentleakratetestholdtime calculatedintheSAR.Thetubeswere thereforeinstalled,andthelidswere placed on QA-hold using EnergySolutions' noncompliance procedure becausetheydidnotmeetthelid configuration authorizedbytheCoCin effectatthattime(Le.,CoC,Rev.20).

EnergySolutions subsequentlymadea determinationthatprior NRC authorizationwasnot required (becausethetubeshadbeenpart of the previous NRC-approved 8-120B caskdesignandwere inadvertently omittedfromthenew secondary lid design), clearedthehold,andreleasedthelidsforuse.Thetubeswere restoredtotheSAR drawingsaspart of a broader license amendment request submittedinMay2014,butthe request was withdrawn for unrelatedreasons.Anewlicense amendment request submittedtotheNRCon 9/16/2015 reintroducesthetubestothe secondaryliddesign.

The presence ofthetubesinthe subject secondarylidtestportshasnosafety consequence because the as-built test volumemeetsthebasisforthe pre-shipmentleakratetestholdtime.

The secondarylidtestport configuration of the subjectlidsis similartothedesign previously authorizedforuse priortoCoC Revision 19.4)Planned Corrective Actions The following corrective actions are planned by EnergySolutions:

  • EnergySolutionshasremoved8-120Bcaskswiththe secondarylidsthathavethetubeinstalledinthetestport(Le., secondary lid numbers 8-120B-5,-6,-7,and-8)from serviceuntiI authorizedtoresumetheiruse.

  • EnergySolutions has evaluated the condition for reportability under 10 CFR71.95and has concluded thatitis reportable.
  • EnergySolutions will notify the licensees (cask users)thathavemade shipments using the 8-120B caskswiththe affected secondarylidsand advisethemto evaluate reportability under 10 CFR 71.95.*EnergySolutions has prepared and submitted an application to NRC to amend CoCNo.9168to include the test porttubeinthefour subject secondarylidsasan authorized configuration.
  • EnergySolutions will perform an apparent cause analysis and identify additional corrective actions to prevent recurrence.
