| Description | Title 10 CFR 50.65(a)(4), states in part, that before performing maintenance activities (including but not limited to surveillance, post maintenance testing, and corrective and preventive maintenance), the licensee shall assess and manage the increase in risk that may result from the proposed maintenance activities. Contrary to the above, on February 17, 2010, the licensee failed to adequately assess and manage the increase in risk associated with maintenance activities in the electrical switchyard. Specifically, the licensee determined that the station had failed to perform an adequate risk assessment for proposed crane activities in the switchyard with regard to Unit 3, which was operating at full power. Before allowing the activities to commence the licensee performed the required risk assessment, and classified the work as a high risk activity in the switchyard for Unit 3, and commenced the crane activity. This was licensee identified because the failure to perform a risk assessment was identified by licensee personnel during an additional final review prior to commencing work. Using Inspection Manual Chapter 0609, Appendix K, Maintenance Risk Assessment and Risk Management Significance Determination Process flowchart 1, Assessment of Risk Deficit, the finding is determined to be of very low safety significance because it only involved risk management actions. The issue was entered into the licensee\'s corrective action program as Nuclear Notification NN 200767351. |
