Safety Analysis

During the time that CH-1B was out of service for repair of the crack in the demineralized flush line, CH-1A and CH-1C were operable except for a two hour period that CH-lC was out of service due to a solenoid that failed open on the leak off line.

Corrective Action

The crack in the weld of the demineralized flush line was repaired by grinding out the old weld and rewelded using a certified procedure.

The line was hydrostatically tested before returning to service.

The vibration in the piping has been addressed and a modific.ation to install suporessors on the pump suction and discharge side has tieen partially completed.

Incidents that are related to this failure were previously documented on LER 78-005,78-006 and 78-046.
