APPENDIX B, continued (Incident Report of Sea Turtle Take)
Draw wounds, abnormalities, tag locations on diagram and briefly describe below.
Description of animal:
At the time of observation at 03:45PM, the specimen was deceased and showing the early stages of decomposition, indicated by foul odor, bloating around the neck and head, missing surface layer of skin in areas of the dorsal surface of hind flippers, and deterioration of the scutes on the carapace (refer to illustration). No obvious damage was observed indicating a potential cause of death. External tags were not observed, and an internal PIT tag was not indicated.

Salem SGS Incidental Take Report - Sea Turtle Attachment 1
Dorsal view of a deceased Kemps ridley sea turtle, as it appeared after retrieval from Salem CWIS intake on 07-21-2022 by Salem Yard crew personnel. Specimen revealed signs of the onset of decomposition, indicated by the missing scutes on the carapace and bloating around the head and neck (07/21/2022).
A ventral view of the specimen, showing the condition of the plastron (07/21/2022).

Incidental Report Sea Turtle Take -Salem Attachment 1
An anterior view, providing a close up of the head (07/21/2022).
The overall round body appearance and the existence of distinct pores near the margins of each of the four inframarginal scutes (identified by the red circles in the photo on both sides of the plastron), verify the identity of the specimen as a Kemps ridley [07/21/22].
