While performing a Reactor Water Cleanup System (RWCU) functional test, a technician lifted the wrong lead causing an isolation of Shutdown Cooling.

The error was immediately recognized, the lead was terminated and Shutdown Cooling was restored.

The wire leads required to be lifted in the performance of certain tests were recently labeled to ensure the identification of the correct lead. The technician had identified the correct lead by examination  :

of the label. He was momentarily distracted however, and in relocating the 1ead chose the incorrect termination modulus. i The procedure instructed the technician to lift lead TB16 (+) to TM30, panel l 1H13-P713D. After identifying the correct lead, he looked away to obtain a  !

l screwdriver, he then proceeded to lif t the thermocouple lead TB16 (+) at TM29. '

The lifted lead caused the RHR A equipment area differential temperature  ;

switch to actuato. The technician immediately terminated the wire.

Operations then restored shutdown cooling to operation. ,

The technician was given a letter of reprimand for the error which was i contrary to the procedure. No hardware or procedural changes are considered necessary. This is submitted'as a final report. l l
