NRC Operating Test Comments & Resolutions Braidwood Station Exam Start Date 11/12/2014 Operating Test 2014

-301 Item Comment Facility Action Response Operating Test Operating Test JPMs RO/SRO In Plant JPMs JPM IP-104: Perform a Local Reactor Trip a) Why is the JPM time critical, what is the basis for the time?

Time critical removed b) Does the JPM have to start in the Shift Office for the time to be measured? No - not time critical c) If the EO is told to perform this step by the US, would EO have to go and locate the procedure or would the procedure be provided?

Procedure would have to be located in the plant next to the trip breakers.

JPM IP-206: Perform Local Reset of Containment Isolation Phase A No Comments JPM IP-409S: Local Start of 1(2)B AF Pump a) If the candidate is starting with step 9, will they use the actual reading on the runtime meter for the first step?

Runtime used.

b) What part of step 4 is really critical? The non

-critical parts should be in their own step.

Critical steps separated.

c) Since the operator has a marked up copy of the procedure should step 7 (procedure step 17) already be marked as N/A?

Step 7 is N/A'ed d) What part of step 8 is really critical? The non

-critical parts should be in their own step.

Steps are now separated.

RO Admin JPMs R-109: Complete NRC Active License Maintenance Log No Comments R-110: Perform Offsite AC Power Availability Surveillance with ACB 1424 control power lost.

a) Task condition 2 should state Unit 1 is at XXX power.

Changed to 100% power b) Why is this time critical?

Critical time removed

-301 Item Comment Facility Action Response c) Step 4. If the applicant does not trace the path on the sheet will mean anything other than incorrect documentation? Why is this critical to trace the lines

? Critical deleted e) Step 7. Separate the critical and non-critical portion of the step.

Done f) Step 9. Separate the critical portion and the non

-critical portions of this step.

DONE g) Step 9. Why is F.3.0 marked N/A in the procedure when the CUE states that it will be completed by another NSO? Is this section to be completed or not?

CUE Removed R-200: Verify Worker Tagout Checklist.

a) Second Evaluator note page 4. Why are there so many different handouts? The candidate should be able to give corrections off of one sheet. Corrected only 1 handout b) Step 4. Why is the tagging sequence incorrect? It is an incorrect sequence for isolating the filter according to procedure. Where does the tag out procedure dictate the filter tagging sequence? If it does not, then this is not critical and there is only on critical step in the JPM. Removed sequence requirement.

c) Are the tags included so that the candidate can review the RI tag content to verify it is correct? I believe that is part of the tag out review requirement for CI and RI tags. This would be a good place to put incorrect information.

Made a different error no tags included. R-403: Perform Offsite Notification (NARS Form Transmittal) for General Emergency a) Package does not contain the form that the candidates will be relaying information from, only a blank form was included.

Fixed during validation.

b) How was 13 minutes determined to be the critical time, why not 10 or 19? Please provide justificati on. Time critical is 10 minutes. SRO portion to 4 minutes to complete document.

-301 Item Comment Facility Action Response c) Step 3: The Note says "Once stops after initial roll call is complete critical time STOPS:," what does this mean? There EPlan procedure counts critical time until roll call is complete.

SRO Admin JPMs S-100: Review Calorimetric Surveillance a) Step 2. Note says that power is 99.51%, the calibration sheets shows 99.47% not quit the same. 99.51% is less than 2% away from average. Changed initial cue to 99.47.

b) Step 2. Make each critical action a separate step.

New step 3 added c) Step 3. Make each critical action a separate step.

New step 5 added d) Step 3. Remove the "notes" and "if asked" cue from the first column. This information is provided in the initial conditions.

Removed S-112: Perform Shutdown Margin Calculation a) The Figures used in the calculation are needed for verification of this JPM. This will be done during the validation.

Done b) Separate all critical action into its own step. Done S-200: Verify Worker Tagout Checklist See comments on R

-100. Done S-300: Review Containment Release for Approval a) Attachment 1 step 5.

e and f. Why would the high alarm setpoint and the Alert setpoint be the same? The high setpoint should be lower by some value. Try 1.82E

-6. After validation setpoint OK b) What is the difference between Step 3&4 and Step 6?

They are carry overs from attachment 1 step 5.e to 5.f. Therefore they are the same critical step. This is also true for C.1.e and f.

After validation steps OK c) Attachment 1 Step 5.f. Unit 1 is not circled.

-301 Item Comment Facility Action Response S-413: Perform Nuclear Accident Reporting System Notification a) File name is S

-410 and the JPM number is S

-413. Corrected b) If this is done in the classroom, have a screen snap shot of the Met data for step 7. If this is done in the simulator, set up the met data so that it can be taken for the machine.

Met data screen printed c) Why are Step 7 and 8 not critical?

Critical d) Should all data critical?

The critical parts are that the form is completed correctly and transmitted correctly. So, 2, all of 4, 7, 8, and 10 are critical steps.

Changed to critical SIM JPMs SIM-102: Perform Movable Control Assemblies Surveillance a) Step 7. What is the basis for DRPI 210 light lit being the failure criteria? A failure has to be justified.

DRPI 210 removed.

b) Step 6. What happens if the candidate moves the rod 3 steps, is that a failure?

3 step removed c) Step 7&8. Break out the parts that are critical in separate steps.

Done SIM-205: Raise SI Accumulator level using a SI pump (NOP) a) Step 9. What is the basis for a candidate failure if the pump runs for more than 30 seconds that is what the trip protection is design to do

? A failure has to be justified.

Removed SIM302: Transfer to Cold Leg Recirculation a) Step 10. What is the basis for the candidate failure if the RWST does reach 9%? The WOG 2 only states that the pumps have to be secured.

9% is in their operations requirement b) There used to be a 30 minute from stopping pumps to starting pumps requirement for Transfer to Cold Leg Recirculation, does this apply to Braidwood?

Does not apply SIM416S: CD/CB Pump Trip No Comments Added note in step 1 to hand procedure to candidate.

-301 Item Comment Facility Action Response SIM510: Perform start of 1A CS Pump for Post Maintenance run a) Step 2. Cue 1 should be "and is being investigated". Why do we need this cue?

CUE 2 start with, "If asked" CUE 3 and 4. Can they verify this on the boards?

Changed event to have the valve closed for maintenance activity and in TS 3.6.6 Added to Initial Conditions b) Step 2.

Is 1CS001A closed so that step 2 is a critical step? Break out the Critical portion for the rest of the step. Valve indication in field SIM608A: Synchronize 1A DG to Bus 141 and respond to Gov. Adj. and overspeed trip failures a) Step 4. What is the basis for the 60 seconds?

A failure has to be justified.

60 seconds removed.

b) Why is this an Alternate Path JPM? Verified Alt path during validation SIM711: Operate a Rad Monitor (Return to service a VCT Cubicle Monito r) a) Step 3. Separate the critical and non-critical steps.

Done SIM801: Respond to RCP Thermal Barrier Leak with CC Valve Failure a) Step 2, last bullet. What is "Responds to Alarm 1 A5, CC SURGE TANK LEVEL HIGH LOW"? It is not in the procedure and there is no reference provided.

Verified procedure Scenarios N145 a) How is Event 7 a Component failure? What actions are taken to mitigate the failure other than an automatic reactor trip?

Changed to part of Major N144 a) Add the phone call for the sodium concentration into a separate Event page for observation.

Done N143 a) 301-5 sheet for crew 1 shows that Event 10 is a Major for the ATC? Correct 305

-1 sheet. Changed OV Comment: Swap event 1 and 2 Done TS Deleted from event 4 Done NRC Operating Test Comments & Resolutions Braidwood Station Exam Start Date 11/12/2014 Operating Test 2014
