Isolated cases were found where some of the east-west spanning runners were not clipped to the north south spanning runners. However, all north south spanning runners are supported from the roof and are prevented from lateral motion. The lights, vents and other items are supported from the structure independently from the ceiling tile system. The overall configuration is consistent with the configuration identified and evaluated in the SEWS walkdown performed on 7/1/93. Due to the fact that the unclipped runners occur only in isolated cases and the fact that all tiles are supported on at least three sides, the ceiling tiles will not fall and adversely impact the panels. There are no ceiling tiles directly over any panels and the panels are all closed on the top. A corrective action report has been issued to reattach the loose runners.9. Some anchorages (welds) of this cabinet are under a vinyl kick plate at its base. The FNP Ops Shift Sup was uncomfortable authorizing the removal of the kick plate due to the potential to bump the cabinet during removal. Review of the anchorages (and of this component overall) were therefore marked Undetermined.

This SWC was originally completed on 8-28-12 and retyped for clarity on 8-27-13. The subject component was removed from the SWEL since the anchorages could not be verified.

This is discussed in the note in Question 6 on pg 1. However, the conclusions of the 8-28-12 walkdown, in response to Questions 7 through 11 above are valid. The Area Walk By for this component, i.e., for Room 416 is documented in AWC package FI-091.Evaluated by: Stephen Yuan kw, A a Date: 08-28-12 08-28-12 Paul A. Instructions for Completing Checklist This checklist may be used to document the results of the Seismic Walkdown of an item of equipment on the SWEL. The space below each of the following questions may be used to record the results of judgments and findings.

Additional space is provided at the end of this checklist for documenting other comments.Anchorag$.

I. Is the anchorage configuration verification required (i.e.. is the item one YZ NO of the 50% of SWEL items requiring such verification)?

2. Is the anchorage free of bent. broken. missing or loose hardware?Cabinet is welded to embedded steel in the floor.3. Is the anchorage fiee of corrosion that is more than mild surface oxidation?
4. Is the anchorage free of visible cracks in the concrete near the anchors?5. Is the anchorage configuration consistent with plant documentation?(Note: This question only applies if the item is one of the 50% for which an anchorage configuration verification is required.)

Anchorage documentation was obtained from SEWS package for this component, dated 7/1/93. Anchorage consists of welds to an embedded plate located on outside of cabinet. Carpeting was partially removed to verify that welds match documented condition and that they have no adverse seismic conditions.
