2. Is the anchorage free of bent, broken, missing or loose hardware? YN NO UC NIAO The anchorageis free from potentially adverse seismic conditions. The paneldoor is missing one of four bolts on outerpanel however this is judged to not be a seismically adverse condition.

The cabinet was identified to contain misc. trash at the bottom of the cabinet. SWEs determined that this trash would have no impact to the equipment during a seismic event and only representeda housekeeping concern. OPS was notified and the miscellaneous trash was removed while the cabinet panel was open.

Second walkdown was performed to inspect cabinet internals. No adverse seismic conditions were noted.

During the IPEEE walkdowns, Q2R21L LOOID was identified as an IPEEE Outlier due to the intemal circuitbreaker supportsystem being anchored to the cabinet with only 2 of 4 fasteners. During walkdown performed on 3/25/13, the circuit breakersupport system was properlyanchored using 4 of 4 fasteners. As a result, it is concluded that the IPEEE Outlier has been resolved.

SEWS package dated 7/2/93 documented 28 1/2" plug welds (4 in each panel) at the embedded steel underneath the front and rear of the load center. Plug welds were also observed attachingthe load center to anotherembedded steel that were not documented in the SEWS package, for a total of 42 plug welds.

Two Agastat relays in upper east panel are mounted with two bolts.

The relays were seismically qualified with four bolts and therefore are missing two bolts. It is evident that four bolt holes were drilled into the panel door, however the relays were mounted too high and only two bolts could be provided. CR 589508 was written to document this condition.

The tall cabinet on the west side of the front of the load center contains a plastic wire chase for the routing of cable. The cover of the wire chase had previously slid down onto a heating element at the base of the panel and had melted. The cover was restored to its original position, but no action was taken to restrainit from future sliding and therefore a potential fire hazard exists during or after a seismic event.

CR589480 was written to document this potentially adverse seismic condition.

The cabinet internalswere inspected and all routed cables have been determined to have adequate flex, routed in an acceptable fashion, and pose no adverse seismic condition.

CR 640948 has been written to document the following condition identified: There were eleven instances identified where seismically qualifiedAgastat Relays were installed with less than the standard four requiredmounting screws. This condition has been evaluated for the 2A MCC, which is attachedto TE 640206, and it has been determined that the currentmounting configurationdoes not adversely impact the functionality of the relay in any of its operatingmodes (before, during, or after a DBE). As a result, the condition does not bring into question the operabilityof the relays. A copy of the evaluation has been attached to the SWC.

Although the relays remain functional and operable, the relays should be installed with the intended mounted configuration of four screws at the earliestopportunity.

The relays associatedbreaker cubicle locations andaffected loads are documented below (reference Unit 2 Load List A351199):

TE-640206 PROBLEM: Eleven (11) relays requiring four screw mounting configurations were found in the field with either only two or three mounting screws.

Demonstrate the technical adequacy of the anchorage and the seismic adequacy of the relays to perform their functions before, during, and after a DBE.

DETAILS: The as-found configurations of the relays are as follows:

four (4) relays were found with three screws, three (3) relays were found with two screws in diagonal corners; three (3) relays were found with two screws in a horizontal line and; one (1) relay was found with two screws in a vertical line.

APPROACH: This evaluation consists of two elements to be verified; (a) structural adequacy of the anchorages, and (b) seismic adequacy of the relays in their as-found conditions.

The evaluation focusses on the assessment of the as-found configurations of 2 screws on a horizontal line and two screws on a vertical line. These assessments envelope all of the subject relay configurations.

ýj ENERCON 05-16-13 TECHNICAL EVALUATION FOR AGASTAT RELAY MODEL #'S E7012AC004 AND E7012AF004 IN MCC 2A (Q2R17B0e01-A) AB EL. 139' ROOM NO. 2332, FNP UNIT 2 IN RESPONSE TO CR# , REF. TE-64e2e6 A. Anchorages were demonstrated to have an interaction ratio of 0.28.

This was based on the accelerations from the horizontal DBE with 5%

damping and vertical DBE with 5% damping amplified with an in-panel amplification factor and a multi-mode factor. The capacity of the screw was compared to the demand on the screws. The multi-mode factor used in the computation was taken from IEEE 344-1975 and the in-panel amplification factor was taken from EPRI NP-7148 and Calculation No.

C891579.

Note: Assessment of the interaction ratio for loads on the screws should normally include the effect of shear in combination with the tensile loads. In this case, since the assessment is very conservative, as in the case of using an amplification factor of 7 for the MCCs (in lieu of an amplification factor of 3), this was neglected. This is reasonable.

8. The relays were demonstrated to operate without chatter based on the computation of the stiffness of the mounting plate with the two most conservative configurations of as-found, existing anchorage. For these cases it is shown that the stiffness of the as-found condition does not contribute to the amplification of the accelerations acting on the relays.

Therefore, the testing conducted in accordance with the referenced seismic report remains valid and applicable to the as-found configurations.

CONCLUSION: The as-found configurations of the eleven subject relays do not adversely impact the seismic qualification of the relays, nor their functionality in any of their operating modes (during, before, and after a DBE).

However, the missing screws need to be re-installed to be consistent with the seismic test configuration and to maintain all design margins.

02R1 7BO98-A was identified as a Seismic Outlierduring the IPEEE walkdowns. The outliers, as documented in the SEWS package for Q2RI 7BO98-A dated 7/5/93 were the following:

1.)MCC has a weld in the rear comers and no otherlocations in the rear.This weld configuration results in an unsupportedspan of 8'4" between welds to resistuplift.

2.) The unsupportedspan in #1 could also be an impact source which could potentiallyresult in relay chatter 3.) 0" gap between back top of MCC and concrete mass on the adjacent wall. This is also a potentialimpact source.

The proposed methods for resolving the outlier was either to attach the MCC top to the adjacent wall (would resolve all 3 concerns listed above) or if modal testing could demonstrate that fn > 8 Hz, then the concrete mass could be removed, which would remove the interactionbetween the wall and the rear of the MCC.

As stated in Attachment E of the SEWS Package,adding anchorageto the rear top of the MCC at the left, center, and right sides of the cabinet would resolve the potential impactsources between the concrete patch and the MCC and the potential impact source as a result of the large unsupported span. The walkdown confirmed that the MCC has been anchored to the wall in the three recommended locations.(Reference Sheets 9, 10, and 11 of 12) Additionally,the concrete mass identified during the IPEEE walkdowns has also been verified to be removed (Reference Sheet 12 of 12). As a result, all IPEEE outliers have been resolved.

The unistrut was inspectedand was found to not be supporting any equipment and was not in contact with associatedequipment, cables, or conduits. A pull test was performed on the unistrut and it was determinedto be adequatelyanchored to prevent any potentially adverse seismic interactions with nearby equipment.

There was some discussionbetween the SWEs about a light fixture directlyabove the valve, but the fixture appearedto be mounted properly. Additionally, with the overall size and materialmake-up of the valve operator,the light fixture poses minimal to no threat of the valve operation duringa seismic event.

During the Area Walk By, there were some houskeeping items such as bags on the floor, fall protection harnesses lying around,and tools lying around. Nearby workers in the area ensured that these houskeeping items would be cleanedprior to exiting the area. Temporary cables were in the vicinity of the valve, but posed no adverse seismic condition. The cables were ensured to be removed prior to the end of the outage. Additionally there were some minor housekeeping items that were to be cleanedby nearby workers.

There were portabletables set up for HP along with phones and phone cables. This equipment is common to be set up during an outage and removed prior to start-up, therefore no CR was warranted. No other seismic conditionsexist that could affect equipment in the area.
