Pursuant to the requirements of 10 CFR 72.212(b)(2), Entergy hereby registers the initial use of the fourteenth, fifteenth, sixteenth, and seventeenth dry fuel storage casks utilized at Waterford Steam Electric Station, Unit 3 (Waterford 3), by providing the following required information:

Licensees Address: 17265 River Road, Killona, LA 70057-3093 Reactor License Number: NPF-38 Reactor Docket Number: 50-382 Cask Certificate Number: 1014 Cask Model Number: HI-STORM 100S Version B Cask Identification Numbers: DFS-837 (initial use June 2, 2015)

DFS-838 (initial use June 16, 2015)

DFS-839 (initial use July 22, 2015)

DFS-840 (initial use July 27, 2015)

Holtec CoC Amendment Number: 5 It should be noted that this notification exceeds the 30 day requirement to notify the NRC for casks DFS-837 and DFS-838. This issue has been entered into the site corrective action program.
